1. Background and rationale

In the Union of the Comoros, the maternal mortality rate increased from 517 per 100, 000 live births [1] in 1996 to 172 deaths per 100, 000 live births [2]in 2012. This positive development could be explained by increased support for prenatal consultation and delivery by qualified personnel. According to EDS-MICS 2012, 92% of pregnant women received prenatal care from qualified health personnel. In addition, according to the same survey, 82% benefited from the assistance of health personnel during delivery and 76% of these deliveries took place in hospitals. Despite the efforts made in maternal health, it is clear that only 32% of women receive a postpartum dose of vitamin A (MICS-EDS 2012). In rural areas, this proportion is 29% compared to 40% in urban areas. With regard to folic acid iron supplementation, we find that 20% of pregnant women do not benefit during pregnancy. One-third (34%) take less than 60 days, 5% for 60 to 89 days, and only 13% for 90 days or more. About six in ten pregnant women (62%) take de-worming medication during pregnancy. It should be recalled that the prevalence of anemia in women of childbearing age is estimated at 30% (MICS-EDS 2012). Many identified socio-economic determinants influence the health status of the mother, including early marriage (31% of women aged 20-49 were married before reaching age 18), clandestine abortions, poor accessibility social and health services, poor quality of services and socio-cultural

With a view to improving maternal nutrition, the Family Health Directorate (DSF) with support from UNICEF is planning to accelerate the implementation of maternal nutrition interventions, especially during pregnancy. However, a situational analysis is necessary. It is within this framework that the Family Health Directorate (DSF) plans to conduct an analysis of maternal nutrition policies and programs and the identification of bottlenecks related to the provision of maternal nutrition services. The present terms of reference specify the purpose of the evaluation and the different tasks to be performed.

2. Objectives

The overall objective is to carry out an analysis of maternal nutrition policies and programs in the Union of the Comoros with a view to identifying key actions to accelerate scaling up of maternal nutrition actions in the context of multisectoral actions. .

Specifically, this consultation will (i) examine the extent to which the policies and directions of the country`s nutrition programs are in line with the 2016 WHO recommendations; (ii) perform a trend analysis of key maternal and child health / nutrition indicators (iii) review coveragematernal nutrition interventions and identify key barriers and facilitators and service utilization platforms; and iv) identify key bottlenecks for delivery and adoption of maternal nutrition interventions.

3. Expected results

At the end of this consultation, the expected results are as follows:

· Bottlenecks related to the implementation of maternal nutrition policies / strategies are identified and analyzed

· The main obstacles related to the implementation of maternal nutrition interventions are identified

· Recommendations for increasing the coverage of nutrition interventions are formulated

5. Main tasks

The analysis of maternal nutrition policies and programs will be conducted at central, island and partner level. It consists of two main components (questionnaires): (a) Analysis of information on existing laws, regulations, policies, standards and guidelines in the area of ??maternal nutrition and (b) Analysis of the main obstacles to the provision of maternal nutrition services during the pregnancy.

Under the supervision of the Deputy Representative of UNICEF Comoros, in close collaboration with the Administrator in charge of nutrition, the consultant will carry out mainly but not exclusively the following tasks:

· Develop a detailed methodology to better support the analysis of information on existing laws, regulations, policies, standards and guidelines in the area of ??maternal nutrition in the Union of the Comoros

· Develop a detailed methodology to better support the analysis of information related to the main barriers to the provision of maternal nutrition services during pregnancy in Union of Comoros

· Organize the collection of information for the 2 questionnaires from the central, island, local and partner administrations

· Draft and validate the 1st draft of the analysis of maternal nutrition policies and programs in the Union of the Comoros. The consultant will host a workshop to present and validate the preliminary results of the analysis of maternal nutrition policies and programs in the Union of the Comoros.

· Present to a select committee the final report of the analysis of maternal nutrition policies and programs after considering feedback and suggestions from the validation workshop.

6. Methodological approach

· The analysis of maternal nutrition policies and programs in the Comoros Union will be conducted at the central, island and partner level. It has 2 main components (questionnaires):

· (a) Analysis of information on existing laws, regulations, policies, standards and guidelines in the field of maternal nutrition

· (b) Analysis of the main obstacles to the provision of maternal nutrition services during pregnancy.

Information on existing laws, regulations, policies, standards and guidelines in the area of ??maternal nutrition will be collected at the central and regional level and from partners.

The Assessment Questionnaire (a) is based on the WHO`s 13 core recommendations on prenatal care for a positive pregnancy experience (Guidelines for a Positive Experience of Pregnancy).

The consultation will use among others the following techniques and methodological aspects:

· A literature review focusing on national policy and strategic documents on maternal and child health, nutrition and nutrition

· Interviews and discussions to collect qualitative and quantitative data that will feed into the situational analysis of maternal nutrition during pregnancy;

The consultant is free to use any other technique that may facilitate the workflow as this methodology has been discussed and agreed with UNICEF and key partners in this process.

The consultant will be required to make a stay of 4 days in Mohéli, 6 days in Anjouan and 7 days in Ngazidja in the field to meet stakeholders and collect information and data at national, regional and district level.

7. Qualification and skills

The qualifications, skills and experience required and desirable to undertake this mission are:

· At least 3 years of professional experience in implementing public health / reproductive health programs, or nutrition

· At least 3 years of experience in developing consultation reports related to public health policies, maternal health and nutrition programs

· Proven knowledge of healthpublic health / reproductive health and nutrition in the Union of the Comoros.

· Demonstrated experience in conducting studies, analyzes, evaluation of public health and nutrition policies or programs in Union of Comoros in the last three years

· Excellent writing and communication skills in French.

· Demonstrated experience and ability to work with multidisciplinary technical teams and / or with an agency of the United Nations System.

· Must have at least a master`s degree in Public Health / Reproductive Health, Nutrition, Health Systems Strengthening or other relevant field.

8. Working conditions

The National Consultant will be based at the level of the UNICEF office and will communicate primarily with the Director General of Health after consultation with the Department of Family Health (DSF), for any decision to be made or guidance to give on the progress of the consultation.

When traveling in the islands of Mohéli and Anjouan, the Family Health Department will, in consultation with its regional branches, make the necessary arrangements to facilitate the conduct of this type of mission in the field.
