Background and Rationale
Like most countries in sub-Saharan Africa, the Republic of Congo is strongly affected by the AIDS pandemic. This is a generalized epidemic. The seroprevalence rate of HIV in the general population is 3.2% with disparities from one department to another ranging between 1.5 and 4.8%. Tuberculosis is a major public health problem in Congo with the incidence of smear-positive tuberculosis cases estimated at 378 per 100, 000 for all forms of tuberculosis. The specific mortality rate is 60 per 100, 000 (World Report TB 2017). HIV seroprevalence among TB patients is 30% in 2016.
The provision of comprehensive care services for PLHIV and TB patients, initially concentrated in the two big cities, Brazzaville and Pointe Noire, has been extended to the interior of the country with the support of the World Bank (WB). and the Global Fund (FM) to fight AIDS, Tuberculosis and Malaria. The total number of functional management sites at the end of 2016 is 70 throughout the national territory, the largest proportions of which are respectively in Brazzaville (18.57%) and Pointe Noire (27.14%). One of the weaknesses in the management of PLHIV in Congo is related to the quality of the data of PLHIV on ARVs and their survival. The active file of PLHIV reported by the National AIDS Control Program (PNLS) does not seem to be consistent with the quantities of ARVs ordered and distributed. The preliminary report of the active file audit conducted in early 2018 revealed that data quality control is not automated at the FOSA level and through higher level half of health facilities do not verify the accuracy, consistency, completeness, and timeliness of the data and half of the FOSA do not benefit from regular supervision. This report also showed that the rate of overestimation of the active queue of PLWHA under routine HAART is 30% at the national level.
The French Red Cross (CRF), a member of the Red Cross / Red Crescent movement, is an independent association that works alongside the public authorities in its humanitarian missions. It has been present in Congo since 1964 with the arrival of a first rescuer instructor and has been involved regularly since 1994 through two ambulatory treatment centers based in Brazzaville and Pointe-Noire. The French Red Cross is working to develop cooperation with the Congolese Red Cross in the fight against HIV and access to water and sanitation.
As part of the operational implementation of the Global Fund 2018-2020 grant for the HIV and TB components in the Republic of Congo, CRf has been selected as Principal Recipient (PR) for both diseases. This grant is implemented from January 2018 for a period of 3 years.
In collaboration with the Ministry of Public Health of Congo, as PR, the French Red Cross is launching this call for applications for the recruitment of an international consultant to support HIV programs (PNLS) and Tuberculosis (PNLT) as well as the Project Operational Unit (UOP) team to set up a digital health data management system.
The present terms of reference define the objectives, the expected results, the work agenda and the profile of the international consultant to be recruited.
Main objective
As part of the implementation of the project funded through the Global Fund 2018-2020 grant, provide technical expertise to the FIU to strengthen the data management system through digital tools.
Specific objective (s) of the evaluation mission
The objectives of the evaluation are:
· To define a data management system via digital tools
· To put in place the defined solutions
· To train the actors of the system
· To monitor and evaluate the implementation
· Provide technical support to the UOP team
With as main tasks:
· Based on the existing data collection system as well as project performance framework indicators, define the database architecture and propose technical solutions for digitized collection and dissemination,
· Develop the database on a digital medium ( KoboToolbox )
· Establish in the health structures and health districts the mobile data collection system using technological devices (tablets) to capture, disseminate and analyze these data,
· Train the CRf team, the health district team and health facility staff in charge of data management on digital data collection and dissemination,
· Provide a technical platform for remote support,
· Monitoring of the mobile data management system.
Conduct of the mission
During the mission, the expert will be contractually supervised by the operations coordinator of the CRf delegation in Brazzaville and the MF grants coordinator at the CRF headquarters in Paris to whom he will report. He will work in close collaboration with the PNLT, PNLS and NHS Department of the Ministry of Health. The CRf delegation in Congo will provide a working environment (office table, etc.). If necessary, He will use delegation logistics as part of the service requirements.
Methodology of work
The evaluator is free to propose his own methodology [in his technical offer]. It is requested that the evaluator carry out his mission in three stages:
· Field mission (s): Meeting with all stakeholders (national level of the Ministry of Health, health district, health structures) to present the objectives of the mission, Evaluate the data collection / transmission circuit, ensure trainings and ensure the proper functioning of the digital health data management system put in place.
· A report to the team of the CRf at the headquarters of the delegation in Brazzaville and the Ministry of Health of Congo.
· The finalization of the consultancy report and its return to the health office of the headquarters of the French Red Cross in Paris.
Expected deliverables
A digital health data management system is in place and functional (more robust and accurate data: entry of data on site, data cleaning limited, intervention logic ...) in all 59 sites supported by the project two months after his mission.
A database of computer data is available at the UOP and the two programs (PNLS, PNLT).
A consultancy report: An evaluation report in initial version will be submitted by the evaluator, at the latest 15 days after his return from his mission. The CRF will make a first reading of the initial report and provide comments. These must be taken into account by the evaluator for the drafting of the final version of his report, no later than 10 days after the feedback.
The report will be clear, concise and unambiguous. It will understand:
· the purpose and purpose of the consultancy
· how to design and conduct the consultancy
· the implementation elements of the specific objectives
· recommendations and lessons learned to ensure the adaptation of the digital data management system.
Qualifications and skills
· Engineer or computer scientist training
· Mastery of KoboToolbox software

Duration of the mission
The mission is planned for an estimated duration of one year. It will be signed a consulting contract of 6 months renewable after an assessment of the services of the consultant.
The schedule indicated by the expert can be reviewed according to climatic hazards and safety conditions.
Monitoring and Coordination
Monitoring and coordination will take place at two levels:
· At the head office of the CRf in Paris: the expert will work in collaboration with the coordinator of grants of the Global Fund and the Health Technical Referent of the Directorate of Relations and International Operations;
· At ground level in Brazzaville:
· With the CRf, the expert will work under the supervision of the coordination of CRF's delegation operations in Congo. It is part of the organization chart of the mission, it respects the security procedures in place, receives specific information on security as any expatriate passing through the country.
· it works in close collaboration with the coordinators and the monitoring and evaluation officers of the PNLS and PNLT.
