3. COMPLIANCE REQUIREMENTS

The Expression of Interest (EOI) should be accompanied by the following documents:

• A cover letter that clearly outlines the type of products/services (from the ones

listed above) the company is offering.

• Clear indication of the business centre and villages being applied for

• Companies must indicate Taxpayer PIN & TIN and Tax Clearance Registration

Number/Certificate number.

• Indicate vendor name as it appears on Public Procurement and Asset Disposal

Board (PPADB) and Companies Intellectual Property Authority (CIPA)

• Company profile with company address, contacts, and banking details.

• List of Directors & shareholders issued by Companies Intellectual Property

Authority (CIPA).

• Certified copies of the Directors & Shareholder-s identity card documents.

• Proven record of previous related assignment with reference letters (at least two)

from clients for each category applied for (applicable for maintenance and works

categories only)

• Key personnel list and contact details should be provided for the particular

category applied for
