Expression of Interest are invited for Development of Continuing Education Modules for Midwives - Congo Brazzaville.

1. CONTEXT

The European Institute of Cooperation and Development ( www.iecd.org ) is an association of French international solidarity created in 1988, whose mission is recognized as being of public utility. IECD operates in 15 countries, particularly in the areas of technical and vocational training, support for small businesses and access of vulnerable people to education and health.

In September 2012, the IECD launched a Health Structures Support Project (PASS), whose objective is to improve the accessibility and quality of care for women and vulnerable children in the department of Pointe-Claire. Black (Congo-Brazzaville). The PASS intervenes in 4 areas:

1. Strengthening the technical skills of medical and paramedical staff;

2. Capacity building for structural management;

3. Community revitalization;

4. Rehabilitation and equipment of some structures.



2. PROJECT DETAILS IMPLEMENTED SINCE 2012



Internal organization :

Partner health centers:

A partnership agreement between the IECD and the Ministry of Health and Population (MSP) of the Republic of Congo was ratified in May 2013.

Since September 2012, the PASS has included in its program 6 health centers (public, private):

o Caritas Medico-Social Work (diocese of Pointe-Noire),

o The Medico-Social Work Anne-Marie Javouhey (Congregation of the Sisters of St Joseph of Cluny),

o The Evangelical Clinic of Mpaka,

o Tié Tié Integrated Health Center

o Tié Tié Base Hospital

o Madeleine Mouissou`s Integrated Health Center

o The Mount Sinai Clinic

o The Loandjili Integrated Health Center

All partner health centers are located in the city of Pointe Noire.



On the specific training component:

The PASS training program has been developed through several stages:

1. Identification of structures and needs analysis through a diagnosis.

2. Definition of an educational approach, based on 2 complementary principles: the skills-based approach (APC) and the action-training.

3. Definition of target audience (s) of the training program.

4. Identification of themes and definition of the objectives of the training program, according to the needs identified in the field.



This resulted, thanks to the mobilization of the expertise of the IFSI-IFAS Virginie Olivier (Paris), to the creation of 3 basic modules for the paramedical personnel (nurses, midwives, laboratory technician, technical health workers, nursery nurses):

1. Hygiene

2. Technical & relationship care

3. Professional values ??and teamwork



In addition, there is a special component to support majors in team management, service management and the notion of responsibility.

These theoretical modules are then supplemented by the support of the staff in their services by the team of trainers, practical workshops on specific points (eg the realization of a simple dressing, the GATPA ...) and an assessment to measure the achievements (and identify training needs).

In parallel and independently of the IECD project, work has been done to update the initial midwifery training program in Congo (skills and training content referential).



Since 2012, the PASS program is:

o More than 30 support documents created and implemented in services for the realization or organization of care (protocols, data sheets, job descriptions, posters ...)

o More than 300 days of theoretical training and 2000 days of support within the services

o Between 57 and 65% of care observed safe to date for the patient against 0% at the beginning of the project



The training program developed, based on basic skills, finds its limits today, especially with midwives. The level of initial training of the latter is very low, and their knowledge of the physiology of pregnancy or childbirth is too weak. Midwives perform gestures and diagnoses routinely without always understanding their meaning or issues. Many monitoring tools (pregnancy, childbirth, postpartum) made available to them are not used because their usefulness is neither mastered nor understood.

In order to increase the impact of the program, the quality and continuity of the care provided, it is now imperative to complement the existing continuing education program. This course, in the form of training modules, combining theory and practice, will integrate into the current course by following the skills approach and adapting to the constraints of the field.



3. OBJECTIVES AND EXPECTED RESULTS

Strengthen the skills and knowledge of midwives in Pointe-Noire health facilities through the development of a continuing education curriculum.



To develop these modules, it will be necessary to answer the various questions about the methodology and organization of the training:

o Essential qualifications / skills expected?

o Training duration ?

o Training methods (classroom training, practical and targeted workshops, individual coaching, supports, syllabus, practical sheets ...)?

o How to evaluate the training (assimilation of content, participation certificate ...)?

o Establishment of follow-up and support for participants after the training?



This work of diagnosis and development of the training modules will be carried out in close collaboration with the midwife project manager present on site, on the basis of existing contents (developed or not by the IECD).



Expected results

1. 1 continuing education course for midwives is developed and its content is created (modules, supports, courses, practical workshops ...). A first version is finalized and tested, it will be rehabilitated according to the conclusions of the first months of testing.

2. The PASS team masters and has appropriated the proposed content and is able to set up training, individual follow-up, regular evaluation.

3. Intermediate reports have been written and describe the steps of this process.
