The objective of this sole source award is for a firm fixed price contract with Radformation, Inc., 335 Madison Ave. Fl. 4, New York, NY 10017, UEI H3PKJDHYM524. This sole source determination is in accordance with FAR 13 Simplified acquisition Procedures in accordance with 41 USC 253(c)(1) Competition Requirements as implemented by FAR 6.302-1, Only one responsible source and no other supplies or services will satisfy agency requirements. Radformation, Inc. is the only authorized servicing agent for the ClearCalc Secondary Calculation and ClearCheck Radiotherapy Treatment Plan Evaluation, Annual Software Subscriptions, and all related equipment and software at the location provided above.
