materials/equipment or labor for the account and official use of the Authority. This Request for Price Quotation or your Formal Bid Proposal maybe

Mr. John Patrick Lazaro at the Office of the Procurement Division on March 14, 2016 @ 2:00pm on which date shall be opened in your
presence or duly authorized representative.

NOTE:
1.QUOTATION SHALL BE SUBMITTED TYPEWRITTEN.
2. ALTERNATIVE BIDS / A BID WITH OPTIONS SHALL BE REJECTED.
3. Request for Quotation shall be obtained from the office of Procurement Division on March 7-14, 2016.
4.Warranty shall be for a period of SIX (6) MONTHS which shall commence after the final acceptance of the end-user.
5. Price validity shall be for a period of 120 calendar days.
6. The LOWEST BIDDER shall be advised to submit sample/s and brochure/s, if applicable, showing specifications of product being
offered and likewise, shall State the Brand in the RFQ or bid proposal.
7. If awarded, the bidder shall submit a Sworn Affidavit of Compliance with the Disclosure Provision of No Relationship and required
registration documents
