Procurement of 4 Units Fogging Machine and 300 pcs NS1 Ag Rapid Dengue Test (RDT) to mitigate dengue outbreak of Mahaplag, Leyte
1. The Municipality of Mahaplag, Leyte, through the MDRRMC Res. No. 10 Series of 2019 and Sangguniang Bayan Res. No. 2019-0051 dated August 7, 2019 to be taken from quick response fund of 5% Calamity Fund for current year 2019 intends to apply the sum of Six Hundred Thousand Pesos only (Php 600, 000.00) being the Approved Budget for the Contract (ABC) to payments under the contract for the Procurement of 4 Units Fogging Machine and 300 pcs NS1 Ag Rapid Dengue Test (RDT) to mitigate dengue outbreak of Mahaplag, Leyte. Bids received in excess of the ABC shall be automatically rejected at bid opening.
2. The Municipality of Mahaplag, Leyte, now invites bids for the Supply and Delivery of Medical Items for the Procurement of 4 Units Fogging Machine and 300 pcs NS1 Ag Rapid Dengue Test (RDT) to mitigate dengue outbreak of Mahaplag, Leyte to be delivered at the LGU - Mahaplag, Leyte. Delivery of the Goods is required within Forty Five (45) calendar days upon receipt of Notice to Proceed. Bidders should have completed, within ten (10) years from the date of submission and receipt of bids, a contract similar to the Project. The description of an eligible bidder is contained in the Bidding Documents, particularly, in Section II. Instructions to Bidders.
3. Bidding will be conducted through open competitive bidding procedures using a non-discretionary -œpass/fail- criterion as specified in the Implementing Rules and Regulations (IRR) of Republic Act (RA) 9184, otherwise known as the -œGovernment Procurement Reform Act-. Bidding is open to all interested bidders, whether local or foreign, subject to the conditions for eligibility provided in the IRR of RA 9184.
4. Interested bidders may obtain further information from the BAC Secretariat, Municipal Hall, Mahaplag, Leyte, and inspect the bidding documents starting November 11, 2019, during office hours except holiday.
5. A complete set of Bidding Documents may be purchased by interested Bidders from the BAC Secretariat, Municipal Hall, Mahaplag, Leyte and upon payment of a non-refundable fee for the Bidding Documents in the amount of One Thousand Pesos only (PhP 1, 000.00). The method of payment will be in cash.
6. The Municipality of Mahaplag, Leyte will hold a Pre-Bid Conference on November 14, 2019, 10:00 AM at the Conference Room, Municipal Hall, Mahaplag, Leyte, which shall be open to all interested parties who have purchased the Bidding Documents.
7. Bids must be delivered to the BAC Secretariat, Municipal Hall, Mahaplag, Leyte, on or before November 26, 2019, 10:00 AM. All bids must be accompanied by a bid security in any of the acceptable forms and in the amount stated in ITB Clause 18.
8. Bid opening shall be on November 26, 2019, 10:00 AM at the Conference Room, Municipal Hall, Mahaplag, Leyte. Bids will be opened in the presence of the Bidders- representatives who choose to attend. Late bids shall not be accepted.
One (1) contract award will be made to a bid meeting the specified requirements. To be eligible for award, the bidder must bid for all items.
9. The Municipality of Mahaplag, Leyte reserves the right to accept or reject any bid, to annul the bidding process, and to reject all bids at any time prior to contract award, without thereby incurring any liability to the affected bidder or bidders.
