Description : Dear Sir / Madam,

We kindly ask you to submit your technical and financial offer as a consultancy assignment: International Consultant for mid-term evaluation, as described in detail in the terms of reference. You will find attached the following documentation:

Annex 1 - Terms of Reference;

Annex 2- Confirmation Interest and Financial Offer;

Annex 2a - P11 Personal Notice

Schedule 3 - Individual Provider and General Conditions;

Annex 4 - Evaluation Criteria.

Your Proposal must include:

Signed Confirmation of Interest and Financial Offer;

CV and / or the Personal Record;

A brief description of why the person considers themselves the most fit to lead the mission, as well as a proposed work plan and methodology on how it will address andcomplete the task;

At least three (3) professional references;

Copy of diplomas.
