7. Copy of Waste Water Discharge Permit of the Septage Treatment and Disposal Facilities.
8. Accomplished Manifest Form.
9. After the Treatment/Disposal of the Waste, Certificate of Treatment from the Accredited Treatment Facility and Certificate of Disposal from the Disposal Facility must be submitted
