The subject of the order is: Delivery of medical equipment and other medical materials: Task 1 - Foam mattress for the Cardiac Surgery Department Task 2 - Anti-decubitus mattress for the Cardiac Surgery Department Task 3 - Bedside cabinet for the Cardiac Surgery Department Task 4 - Trolley toilet and shower for the Cardiac Surgery Department. The Ordering Party allows the submission of partial offers. Parts of the order are individual tasks specified above, i.e. tasks 1 to 4. The Ordering Party informs that the Contractor may submit an offer for a maximum of 4 tasks (without limiting the number of tasks per one Contractor). A detailed description of the subject of the order is included in Annex 2b and 4 to the SWZ 3.3. The Ordering Party's requirements regarding the subject of the order: 1) the offered medical device must have approvals, certificates and be registered in accordance with legal provisions requiring approvals, certificates and registration, 2) the offered medical device must be placed on the market and used in accordance with the requirements of the Act of April 7, 2022. on medical devices (Journal of Laws of 2022, item 974), 3) the offered medical device must meet the standards or technical specifications in force for this product; If the offered medical product does not require notification to the President of the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products, the Contractor is obliged to perform, at its own expense and on its own, all activities necessary to fill...
