The letter of application (form DC1 duly completed and signed) certifying that it does not enter in any of the cases mentioned in the code of the public order relating to the prohibitions to tender to the public markets and that it has satisfied its fiscal and social obligations. The candidate`s declaration (form DC2 duly completed). In casegrouping, each member will have to provide DC2; The forms DC1, DC2 are downloadable on http://www.economie.gouv.fr/directions_services/daj/marches_publics/formulaires/index.htm - the preceding documents can be replaced by the Single European Market Document (DUME) established in accordance fixed in the regulation of the European Commission.

- value of work services: 5%: sub-criteria:

- level of description of the services-materials and richness of the proposal: 25%,

- ergonomics of the proposed implementation from the point of view architectural: 25%,

- technical requirements and impact on ancillary premises: 20%,

- impact of the building site on the activity of the hospital and time of realization: 30%.

Development conditionssustainable: 3%. Financial conditions: 40%. The award criteria and their possible sub-criteria are detailed in the regulation of the consultation.
