The subject of the contract is the supply of medical equipment for Multifunctional Oncologyinterventional angiological workplace. Services related to installation and transport to the destination (connected with unloading of goods, disposal of packaging) are also part of the subject of the contract

Brief description of subject of contract is given in point II.1.4) of this notice

1.1 The applicant must meet the conditions for participation specified in - 32 od. 1 Act no. 343/2015 Coll. on Public Procurement and on the Amendment of Certain Outcomes as amended (hereinafter referred to as the "Act"). Pursuant to Section 32 (1) (e) of the Act, the tenderer must prove that he is entitled to deliver the goodswhich are subject to this contract. At the same time, the criminal integrity of a legal person is required, according to Act no. 91/2016 Coll. on Criminal Liability of Legal Persons and on Amendment and Supplementation of Some Conclusions

1.2. 2 pism. e) is prohibited by reason of the use of data from public administration information systems pursuant to a special regulation (Act No. 177/2018 Coll.reducing administrative burdens by using public administration information systems and on amendments to certain outcomes (outcomes against bureaucracy) need not be presented. In the case of participation conditions pursuant to - 32 para. 2 pism. e) the above rule applies to economic operators (interested parties) listed exhaustively in - 2 para. 2 Act No. 272/2015 Coll. on the Register of Legal Persons, Entrepreneurs and Orpublic authorities and about the amendment of some conclusions. In other cases the candidate is still obliged to submit a document proving the fulfillment of the condition of participation in personal status (eg an extract from the Commercial Register or the Trade Register).treasures according to - 32 par. 1.4 The contracting authority shall recognize an equivalent entry, such as entry in the list of economic operators under the law or a certificate of entry issued to the competent authorities of another Member State, by which the tenderer proves that the conditions for participation in the tendering procedure have been fulfilled. The contracting authority shall also accept other equivalent documents submitted by the tenderer.to pay documents proving the fulfillment of the conditions of participation determined by the contracting authority by a single European document within the meaning of - 39 of the Act. Demonstration of the conditions of participation is also done to the contracting authority in accordance with - 152 para.

1.6 Group of suppliers demonstrates fulfillment of conditions of participation in public procurement concerning personal status for each member of group of personsITnet. Fulfillment of participation requirement according to - 32 par. 1 pism. (e) a member of the group proves the law only in relation to the part of the subject of the contract which he / she has to arrange for me. 2 of the Act and pursuant to - 34 par.

Conditions of participation in public procurement pursuant to - 33 of the Actfinancial and economic position:

2.1 - 33 par. 1 pism. a) of the Act - statement of the bank or branch of a foreign bank,

2.2 - 33 para. 1 pism. d) law - summary of the total turnover for the last 3 marketing years, for which they are available depending on the establishment or commencement of the operation of the activity. 1 et seq. for

4.VO recommends that applicants download the tender documents (SP) only after they have registered with the Eranet system. The contracting authority does not have any records of candidates who will receive the tender documents without registration - I will not be informed about the publication of additional information and explanation of the tender documents.

5.For the needs of electronic communicationEach candidate / applicant is obliged to register in the Eranet system at the URL: https://nou.eranet.sk. The applicant is only entitled to submit a tender after registering and logging into the Eranet system. Registration for Eranet is free of charge

7.The submission of offers is carried out exclusively electronically, via the Eranet system available at the following address: https: //nou.eranet.sk.

8.ExplainedNot with the information necessary to draw up the tender and to demonstrate compliance with the conditions for participation, the PO shall notify without delay at least 6 days prior to the expiry of the deadline for submission of tenders, provided that the interested party requests sufficient explanation in advance. III.1.2) Economic and financial position and III.1.3) Technical and professional capacitynetwork and the method of their demonstration are stated in Annex no. 5 SP. A specimen of the single European document is annexed no. 3 SP.

10.A tender security of EUR 20 000, 00 is required. The method and conditions of the security shall be determined by law and detailed in point 11 of the CM. I will prove that the security has been deposited in the bids. The security must be credited to the designated VO account or presented in for the bank guarantee in the tenderer's bid no later than the deadline for submission of tenders. The NA will classify the failure to meet this requirement as a failure to meet the conditions set by the ROI and exclude such tenderers. The account number and the bank constituting the security are listed in the relevant section of the SP (point 11.5 (a)). In the case of security deposit by bank transfer, we recommend that the document provesstated fact by the part of the offer. In the case of collateral security in the form of a bank guarantee, resp. in the form of a guarantee insurance, the document must be indicated by the tender part and its validity can be proven during the whole period of the tendering. on timeThe contract is not subdivided because the subject - matter of the contract is homogeneous in terms of function, time, locality, technical specifications and the type of contract concluded. Social aspects, green public procurement, procurement of innovations were not applied - d) none of the above.
