O thighsContract greens may apply to contractors who:

1) applies to lot no. 1-10: have a registered business activity in the field of production, storage, packaging and marketing of agri-food products, in accordance with the Act of December 21, 2000 on commercial quality agri-food products,

2) applies to all parts: they have notified activities subject to official controlli organs of the State Sanitary Inspection, in accordance with the Act of 25 August 2006 on food and nutrition safety, or have registered activities related to the production, processing and distribution of animal products, in accordance with the Act of 16 December 2005 on products of origin animal.

The Employer sets the minimum requirements for the abovementioned wamarket:

Contractors who have adequate civil liability insurance can apply for awarding the contract, i.e. they are insured against civil liability (tort, contract) in the scope of conducted activities related to the subject of the contract for a guarantee sum of at least:

a) PLN 500, 000 in the case of submitting an offer for Lot 1 - Supply of fresh vegetables and fruitw and seasonal fruit

b) PLN 390, 000 if submitting an offer for Lot 2 - Deliveries of milk and milk products

c) PLN 280, 000 if submitting an offer for Lot 3 - Deliveries of general purpose food

d) PLN 70, 000 for submitting an offer for Part 4 - Deliveries of food for special nutritional purposes (baby food)

e) PLN 280, 000 inin the case of submitting an offer for Lot 5 - Deliveries of meat and sausages

f) PLN 180, 000 if submitting an offer for Lot 6 - Deliveries of frozen vegetables and fruit

g) PLN 70, 000 if submitting an offer for Lot 7 - Deliveries of frozen fish fillets

h) PLN 30, 000 if submitting an offer for Lot 8 - Deliveries of fresh fish fillets

i) PLN 60, 000 if submitting an offer for Lot 9 -Deliveries of freshly baked sourdough bread

j) PLN 27, 000 if submitting an offer for Lot 10 - Delivering chicken eggs

k) PLN 25, 000 when submitting an offer for Lot 11 - Deliveries of natural spring water together with a lease and service of heated dispensing dispensers and chilled natural spring water from containers

Compliance with the condition of participation in the procedure must be demonstratedSeparately for each part for which the offer is submitted (in case of submitting an offer for more than one part of the contract and wanting to present the same liability policy for all parts of the contract, the condition of participation in the procedure specified for these parts should be added together, however, the Employer does not require the presentation of the policy Civil liability insurance for a sum of more than PLN 1, 000, 000, regardless of how muchThe value of the part of the order for which the offer is submitted.

The values provided in the documents confirming the fulfillment of the condition in currencies other than PLN, the Contractor will calculate according to the average exchange rate of the National Bank of Poland (Table A) on the day of the deadline for submission of tenders.

1. List of documents that the Employer requires to submit, together with the offer:

1) bid form

2) price calculation - for a given part

3) proof of paymentdium - for a given part

4) photocopies of labels of articles covered by the quality system

5) power of attorney - if applicable

7) EAT declaration

2. Contractors who are not subject to exclusion pursuant to art. 24 paragraph 1 points 12-23 and art. 24 paragraph 5 points 1-8 of the PZP and for preliminary proofs it is necessary to present: axis in the form of a single document, i.e. EAT.

3. Before giving fromam, the Contractor whose offer has been rated the highest, will be called upon to submit, within not less than 10 days, the following documents valid as at the date of their submission:

1) in order to confirm that there are no grounds for exclusion, the Employer requires submitting the documents and statements referred to in point 9.3.1 of the ToR in accordance with - 5 of the Regulation. Minister of Development of 26.7.2016 on documents, whichcan be requested by the contracting authority from the contractor in the procurement procedure (Journal of Laws of 2016, item 1126, as amended).

2) to confirm the circumstances referred to in art. 25 paragraph 1 item 1 of the Public Procurement Law:

a) in the entity authorized to control the quality of monsters, that the delivered products meet specific standards or technical specifications, and in the event that the Contractor does not have any information that offersagri-food articles he meets meet the requirements of commercial quality specified in the provisions on commercial quality and additional requirements for these articles if their fulfillment has been declared by the manufacturer

b) in the decision or opinion or opinion of the competent authority of the State Sanitary Inspection or Veterinary Inspection supervising the application of the principles ofdifferent HACCP system or Good Manufacturing Practice (GMP) or Good Hygienic Practice (GHP) principles. The Employer allows the presentation of another document, i.e. a current certificate of an independent entity (certification body accredited by the Polish Center for Accreditation) confirming the implementation of the HACCP system or Good Production Practice (GMP) / Good Hygienic Practice principlesj (GHP).

c) Photocopies of the labels of the items offered or other documents containing information directly from the manufacturer and signed by him on the subject of the contract in the scope relating to its inclusion in the quality system. For the evaluation of the offer in the "Quality System" criterion, the Employer will take into account only the documents submitted in the offer on the expiry datenu submitting bids (the above documents under the bid evaluation criterion are not subject to supplementation pursuant to Article 26 (1) or Article 26 (3) of the PZP).

d) In the case of submitting an offer for lots No. 6 and 7, photocopies of labels (for each from the item price calculation) offered articles or other documents (product specifications) containing information directly from the manufacturer about the offered itemorder details to the extent related to the Employer`s requirements set out in the description of the subject of the order for a given part. or newer in the production / bottling of the spring water offered by the Contractor.

f) In the case of submitting an offer for lot No. 11 Certificate issued by the National Institute of Public Health National Institute of Hygiene containing the assessment and qualification of the water offered; the water offered must meet the requirements set out in the Regulation of the Minister of Health of 31.3.2011 on natural mineral waters, source waters and table waters (Journal of Laws of 2011 No. 85, item466).
