The concept of arranging green areas along streets with an executive design of plantings and accompanying changes within the boundaries of road lanes of selected streets in Warsaw.

The subject of the order is the development of the concept of land developmenturban greenery along the streets with an executive design of plantings and accompanying changes within the road borders of selected streets in Warsaw.

The subject of the order is the concept of greenery along selected streets, inventory of development with special regard to greenery, planting documentation and multi-branch executive documentationchanges in land development in the road lane and neighboring areas, as well as author's supervision over the implementation of work carried out on the basis of documentation.

1. Each contractor must attach to the offer the Single European Procurement Document valid as at the day of submitting the offersia. The information contained in the statement will constitute preliminary confirmation that the contractor is not subject to exclusion from the proceedings and meets the conditions for participation in the proceedings. The uniform document is Attachment 6 to the ToR.

NOTE: In Part IV: Eligibility Criteria, the Contractor may confine himself to completing the alpha section in Part IV of the Single Document and does not have to fill in any of the remainingsection (A-D) in part IV.

2. The Contractor who refers to the resources of other entities in order to demonstrate the absence of grounds for exclusion against them and fulfill - in the scope in which they refer to their resources - the conditions for participation in the proceedings also consist of uniform documents relating to these entities

3. In the case of joint application for contractors, a single documentis submitted by each of the contractors jointly applying for the order. These documents confirm the fulfillment of the conditions for participation in the proceedings and the lack of grounds for exclusion to the extent that each of the contractors demonstrates fulfillment of the conditions for participation in the proceedings and the lack of grounds for exclusion. The contractor is excluded from the contract for any of the contractorscircumstances referred to in art. 24 sec. 1 and par. 5 points 1, 2 and 4 Public Procurement Laws

5. The contracting authority will call the contractor whose offer has been rated the most, to submit within the prescribed, not shorter than 10 days, the current date as of the day of submitting the following declarations or documents:

5.1 to confirm that the contractor meets the conditions for participation in the proceedings:

5.1.1 list of services - inzor list is set out in Annex 5 to the ToR,

5.1.2 of the list of persons - template list is specified in Annex 6 to the ToR.

5.2 to confirm the absence of grounds for excluding the contractor from participation in the proceedings:

5.2.1 information from the National Criminal Register within the scope defined in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submission of tenders;

5.2.2 educationcontractor's failure to issue a final court judgment or final administrative decision against him or her, whether or not he pays taxes, fees or social security or health insurance premiums or - in the event of such a verdict or decision - documents confirming the payment of such claims together with any interest or fines or the conclusion of a binding agreementproperty on the repayment of these receivables;

5.2.3 the contractor's statement about the lack of a ruling against him as a preventive measure for the prohibition of applying for public orders;

5.2.4 extract from the relevant register or central records and business information, if separate regulations require entry in the register or records.

6. To confirm the absence of grounds for exclusionthe perpetrators from participation in the proceedings in the circumstances referred to in art. 24 sec. 1 point 23 of the PPL Act, the Contractor within 3 days from the date of publication
