Supply of equipment for two laboratories in the Special School and Educational Center No. 1 as part of the project "Modernization of vocational education in Elbląg Stage II"

Subject of the order is the supply of equipment for two laboratories in the Special School and Educational Center No. 1 with the possibility of submitting partial offers for individuale tasks:
Scope Parts 1 orders includes:
A. Delivery of equipment to the Training Workshop for the cook profession;
Detailed description and quantity of equipment is included in the ANNEX No. 1a to the ToR.
B. assembly;
C. commissioning and instruction;
D. labeling of equipment in accordance with the "Handbook of the applicant and beneficiary of the 2014-2020 policy programs for information and promotion";
E.-during the warranty period for equipment that requires servicing.
The ordering party informs that it does not provide for the order referred to in art. 67 sec. 1 point 7 of the Public Procurement Law.
C.d. the entries from section VI.3): - the Contractor having its registered office in the territory of the Republic of Poland, in relation to a person residing outside the territory of the Republic of Poland, to whomthe document indicated in section VI.3), item 6, item 1), submits the document specified in - 8 of the Regulation of the Minister of Development of 26.7.2016 on the types of documents that the contracting authority may request from the contractor in the procurement procedure (Journal of Laws of 2016, item 1126), to the extent specified in art. 24 sec. 1 point 14 and 21 of the Public Procurement Law - if the Contractor is established or resides outside of the territorytorium of the Republic of Poland, instead of the documents referred to in Section VI.3), item 6, item 1) -4) submits the documents referred to in - 7 of the above-mentioned ordinance.

2. Grounds for exclusion: art.-24 sec. 1 and 5 points 1 and 8 of the Public Procurement Law - 3. For the purpose of preliminary confirmation that they are not subject to exclusion and meet the conditions for participation in the procedure, the Contractors shall submit together with the offer:
1) Uniform European Order Document (EAT);

1) Declaration in the scope of art. 25 ust. 1 point 2 of the Public Procurement Law.
6. In order to confirm that it is not subject to exclusion, -The contractor, whose offer has been rated the highest, submits to the ordering party:
1) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids;
2) certificate of the competent head of the tax office confirming that the contractor is not in arrears with paying taxes, not earlier than 3 months before the deadline for the submission of bids, or other document confirming that the contractor has entered into an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, in particular has obtained the legally required exemption, postponement or distribution of overdue installments payment or suspension in full of the decysy competent authority;
3) a certificate from the relevant local organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of tenders, or other document confirming thate the contractor has entered into an agreement with the competent authority regarding the repayment of such receivables, together with any interest or fines, in particular obtained legally permissible, postponement or payment of overdue installments or suspension in its entirety of the competent body's decision; - 4) excerpt from the relevant register or from central records and information about businessif separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 1 of the Public Procurement Law Act,
5) the contractor's statement on the lack of issuing a final court judgment or final administrative decision on default of taxes, fees or social security or health insurance premiums foria of such a judgment or decision, documents confirming payment of such claims together with any interest or fines or conclusion of a binding agreement on repayment of such receivables; 6) declaration of the contractor about the absence of a ruling against him as a precautionary measure prohibiting the application for a public order;
7) the contractor's statement about not paying the taxin and local fees, referred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2016, item 716).
