The subject of the order is realizationtasks Expansion of domain systems and implementation of e-services for residents along with the supply of IT equipment and instructional videos as part of the project "Modern e-office in Dzierżoni-w", as detailed in Annex 1 to the ToR.

1. The subject of the order is the implementation of the task called Development of domain systemsand the implementation of e-services for residents along with the supply of IT equipment and instructional videos as part of the project "Modern e-office in Dzierżoni-w", as detailed in Annex 1 to the ToR.

The task is co-financed by the European Union, with of the funds of the European regional development fund under the Regional Operational Program of the voivodeLower Silesia 2014 2020.

2. The subject of the contract includes:

1) implementation of 11 e-services at the level of maturity 4 and more and 16 services at level 3 (including the implementation of the e-journal in 4 primary schools) and purchase of IT equipment (retrofitting the server room with 2 servers, UPS, system for virtualization, operating system for servers) and software,

2) creating a design platform, connectingj all e-services offered and enabling easy access to services for clients, modernization, integration of domain systems, spatial information systems with the EPUAP system and delivery of new IT systems,

3) extension of domain systems to handle local taxes and fees, and property management with new functionalities, thanks to which implementation will be possibleie new e-services at least at the 4th level of maturity (transaction),

4) launching the system,

5) training employees of the City Council in Dzierżoni-w, councilors of the City Council Dzierżoni-w, school employees,

6) granting the Ordering Party a license to use the works described agreement,

7) warranty for the system,

8) making instructional videos.

3. Software delivered / createdAs part of the system, it must have a modular structure that implements individual functional groups using autonomous components. The component integration function must be implemented by a common application platform via standardized interfaces in accordance with the Regulation of the Council of Ministers of 12.4.2012 on national interoperability frameworks, minimum requirementsfor public registers and exchange of information in electronic form and minimum requirements for ICT systems (i.e. Journal of Laws of 2017, item 2247).

4. The contractor is obliged to carry out the contract under the terms and conditions described in the contract template constituting Annex 8 to the ToR.

5. The Contractor undertakes to conclude a contract with the Employer for entrusting data processingpersonal data in accordance with the specimen constituting Annex 9 to the ToR.

Employer in accordance with art. 24aa section 1 of the Act will first evaluate the bids and then examine whether the Contractor whose bid was assessed as the most advantageous is not subject to exclusion fromonce meets the conditions for participation in the proceedings. The offer, statements, including EAT, lists and documents are submitted in electronic form and signed with a qualified electronic signature. The method of submitting the offer and documents, including the encryption of the offer, is described in the instructions for use, Annex 12 to the ToR.

The offer must contain:

1. Documents submitted by all Contractors:

1) attachdisappear No. 2 to the Terms of Reference for the properly filled in "Contractor's Bid" form. The Contractor is required to complete this form regardless of the completed bid form on the platform: https://dzierzoniow.ezamawiacy.pl/;

2) Single European Order Document Form (EAT), drawn up and completed in accordance with the template set out in Commission Implementing Regulation (EU) 2016/7 of 5.1.2016 establishing a standard form for the single European procurement document (OJ EU L 2016, No. 3, p. 16);

3) The Contractor, which relies on the technical or professional abilities of other entities, irrespective of the legal nature of the relations between them, makes an undertaking in accordance with the principles set out in the ToR and the Act. The template of the commitment is forconnector No. 3 to the Terms of Reference;

4) if the Contractor, demonstrating compliance with the conditions referred to in art. 22 paragraph 1b of the PPL, relies on the resources of other entities on the principles set out in Art. 22a paragraph 1 Public Procurement Law, the Contractor is obliged to: submit a third party statement on the fulfillment of the conditions for participation in the procedure (in terms of the condition in relation to which it provides its potential) and the lack of grounds forfor exclusion submitted on the EAT form in accordance with the provisions of point VII of the ToR;

5) power of attorney to represent the Contractor, if the offer is submitted by an attorney in the original in electronic form signed by an electronic signature qualified by authorized persons or by a notary confirmed copy for compliance with the original in electronic form.

2. Documents submitted by all Contractorsow:

The Contractor within 3 days of placing on the purchasing platform and on the website http://bip.um.dzierzoniow.pl/ information from the opening of the offers referred to in art. 86 section 5 of the Act, provides the Ordering Party via the purchasing platform a declaration of belonging or not belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the Act. withby submitting a statement, the Contractor may provide evidence that links with another Contractor or Contractors who have submitted bids do not lead to distortion of competition in the procurement procedure. The information should be prepared in accordance with Annex 5 to the ToR.

3. Documents submitted only by the Contractor, whose bid will be highest rated:

The Contractor submits via intermediationem purchasing platform at the request of the Employer within a specified period (not less than 10 days) valid as at the date of submitting the statement or documents confirming the circumstances referred to in art. 25 paragraph 1, that is:

1) the documents listed in point VI.2 lit. a to g SIWZ;

2) documents and statements confirming the fulfillment of the conditions for participation in the procedure in connection with the provision of their resourcesobligated by another entity (if applicable). They should be submitted in the original in the form of an electronic document or in an electronic copy of the document or statement, certified as being true by the entity providing the resources.
