Digitization of selected collections of the Czartoryski Princes Library as part of the project"Heritage of Polish Music in Open Access". A detailed description of the subject of the order is included in Annex 2 to the ToR.

Digitization of selected collections of the Czartoryski Library as part of the "Heritage of Polish Music in Open Access" project. A detailed description of the subject of the order containsAnnex 2 to the ToR.

The contracting authority will exclude the Contractor from the procurement procedure in relation to which any of the circumstances referred to in art. 24 paragraph 1 point 12 23 of the Act (mandatory grounds for exclusion) and in Art. 24 paragraph 5 point 1 and point 8 of the Act (optional grounds for exclusion). List of statements or documents confirming compliance with the conditions for participationin proceedings, no grounds for exclusion specified by the Employer:

1) single European order document (EAT);

2) if the Contractor relies on the financial or economic situation of other entities to confirm compliance with the conditions of participation in the procedure, he is required to submit evidence that the contract will have the necessary resources of these entities, in particularthe obligation of these entities to make available to him the necessary resources for the purposes of the contract (if applicable);

3) the Contractor`s statement on belonging or not belonging to the same capital group;

4) information from the National Criminal Record in the scope specified in art. . 24 section 1 item 13, 14 and 21 of the Act, issued not earlier than 6 months before its expirydeadline for submitting bids;

5) a certificate from the competent head of the tax office confirming that the Contractor is not in arrears with payment of taxes, issued not earlier than 3 months before the deadline for submitting bids or other document confirming that the Contractor has concluded an agreement with the competent tax authority regarding the repayment of these receivables with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or installment of overdue payments or suspension of the entire implementation of the decision of the competent authority;

6) certificate of the competent organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with payment insurance premiumssocial or health baking issued no earlier than 3 months before the deadline for submission of bids, or other document confirming that the Contractor has entered into an agreement with the competent authority on the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption, deferral or installment of overdue payments or will suspendnot in full implementation of the decision of the competent authority;

7) an excerpt from the relevant register or from the Central Register and Information on Economic Activity, if separate provisions require an entry in the register or register;

8) the Contractor`s statement that he has not issued a final or final judgment against him administrative decision on late payment of taxes, fees or insurance premiumssocial or health care or in the event of a judgment or decision being issued, documents confirming the payment of these debts together with any interest or fines, or the conclusion of a binding agreement on the repayment of those debts;

9) the Contractor`s declaration of non-payment of local taxes and fees, for which referred to in the Act of 12.1.1991 on taxesand local fees;

10) a statement by the Contractor that no decision has been issued against him as a precautionary measure forbidding public procurement. If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland instead of the documents referred to in items 4, 5, 6, 7 above, he shall submit the documents specified in par. 7 of the Regulation of the Minister of Development of 26.7.2016 on the types of documentsntow that the Employer may request from the Contractor in the procurement procedure (Journal of Laws of 2016, item 1126). Ww. the regulation applies to the Contractor based in the territory of the Republic of Poland, in relation to a person residing outside the territory of the Republic of Poland (paragraph 8 of the Regulation). The contractor remains bound by the submitted offer for a period of 60 days.
