The subject of the order is the purchase of equipment for the Monoprofil Center of Medical Simulation of the State Higher Vocational School. prof. Edward F. Task 8 - Delivery phantom

The detailed scope of the order is described in Annexes 2.1 - 2.8 to the ToR.

The Ordering Party does not set specific requirements to meet this condition.

The condition in the understanding of the Employer will be fulfilled by the Contractor who is insured against civil liability in the scope departmentrelated to the subject of the order for a guarantee sum of min. PLN 500, 000.00 (five hundred thousand).

List of statements or documents filed by the Contractor in the proceedings at the request of the Employer to confirm the circumstances referred to in Article. 25 ust. 1 point 1 of the Act concerning the economic or financial situation: in order to confirm the fulfillment of the condition regarding the economic situationor financial ordering, the Employer requests an insurance policy or other document confirming that the Contractor is insured against civil liability in respect of the business related to the subject of the order for a guarantee sum specified by the Employer.

The Purchaser informs that pursuant to Art. 24aa paragraph 1 of the Act, it will first evaluate the offers and then go onwhether the contractor whose tender has been rated as the most advantageous is not subject to exclusion and meets the conditions for participation in the procedure. The Contractor will provide a bid bond in the amount of 20, 000.00 PLN (twenty thousand). The bid bond can be contributed in one or more of the following forms:

a. money;

b. bank guarantees or social-savings credit sureties, except that aftercash check is always a financial guarantee;

C. bank guarantees;

d. insurance guarantees;

e. sureties granted by entities referred to in art. 6b par. 5 point 2 of the Act of 9.11.2000 on the establishment of the Polish Agency for Enterprise Development (Journal of Laws of 2018, item 110).

The bid bond paid in cash is paid by bank transfer to the bank account of PWSZ in Suwałki 7212403363 1111 0010 5218 1015 by 27.8.2018 to the following hours 09:00 (the date of the influence of the funds on the account of the ordering party decides). A photocopy of the proof of payment of the bid security should be submitted in the offer.

The bid security in the remaining forms of the original is made up in the State Higher Vocational School prof. Edward F. Szczepanik in Suwałki, room 203, ul. Noniewicza 10, 16-400 Suwałki before expiry-deadline for submission of tenders.

Each contractor must submit in the electronic form with a qualified electronic signature, valid until the date of submitting bids, the declaration referred to in art. 22 of the Act, submitted in the form of a Single European Order Document (related to - "EAT") which specimen specifies the standard form specified in the Regulationthe European Commission issued on the basis of 59 par. 2 of Directive 2014/24 / EU. For the purpose of preliminary confirmation that it is not subject to exclusion and meets the conditions for participation in the proceedings. If the Contractor refers to the resources of third parties in order to demonstrate compliance with the conditions for participation in the proceedings, he shall submit to the third party. EAT should becompleted in the scope in which the Contractor uses the resources of the third party and relate to verification of the grounds for exclusion. If the Contractor intends to entrust the execution of a part of the order to subcontractors, he is obliged to complete part II of section E EAT, including, if known, subcontractors. EAT in α part IV of the form requires submission by the Contractor general

Contractor, within 3 days of placing on the order information website referred to in art. 86 ust. 5, gives the Ordering Party a declaration on belonging to or not belonging to the same capital group. Together with the declaration, the Contractor may provide evidence that links with another Contractor do not lead to distortion of competition in the proceedings. no capital group may submit a statement together with the offer.
