Supply of consumables for renal replacement devices owned by the Employer and consumables for operations performed in continuous techniques, along with the equipment rental - Reference Number:ZP 39/19 - The subject of the order is the supply of consumable materials for replacement-replacement devices owned by the Employer and consumable materials for treatments performed in continuous techniques together with equipment rental, assortment and quantities specified in the Price Form constituting Annex 2 to the SETC.

The subject of the order is the ma deliveryconsumable materials for replacement-replacement devices owned by the Employer, about the assortment and quantities specified in the Price Form constituting Annex No. 2 to the ToR.

I. Basics of Exclusion:

Art. 24 sec. 1 item 13-23 and art. 24 sec. 5 point 1 Pzp. The initial confirmation / confirmation of the absence of grounds for exclusion is the Contractor`s validity for the tender valid as at the day of submitting bidsnot art. 25a para. 1 Pzp in the form of a Standard Form of a Single European Order Document.

List of declarations / documents to be provided at the request of the contracting party to the contractor whose offer has been rated the most, in order to confirm the lack of grounds for exclusion. Information from the National Criminal Register in the scope specified in art. 24 sec. 1 items 13, 14 and 21 Pzp - Presentednot earlier than 6 months before the deadline for submission of tenders.

2. Declaration of the contractor about the lack of issuing a final court judgment or final administrative decision on default of payment of taxes, fees or social security or health insurance premiums, or - in the event of such a verdict or decision - documents confirming the payment of these claimswith any interest or fines or a binding agreement on repayment of these debts.

3. Declaration of the contractor about the lack of a decision against him as a preventive measure for the prohibition of applying for public orders.

4. Contractor`s declaration of affiliation or lack of belonging to the same capital group. Contractor, within 3 days of placing on ston the internet information referred to in Article 86 par. 5 PPL, he provides the orderer with a declaration of belonging or non-affiliation to the same capital group, referred to in art. 24 sec. 1 item 23 of the PPL, with the submission of a statement, the contractor may provide evidence that links with another contractor do not lead to distortion of competition in the procurement procedure.5. A copy from the relevant register or from the Central Register and Information on Economic Activity, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Article. 24 sec. 5 point 1 of the PzP. - Information on the joint application of contractors for the order and in the case where the contractor or place of residence of the contractor isthey are located outside the territory of the Republic of Poland have been described in the content of the ToR.

II. The Contracting Authority demands a bid bond in the amount of PLN 29, 000.00 for the entire proceedings, the amounts for individual packages have been indicated in the ToR and in Section II - Information on Parts.

III. The offer validity period is 60 days from the date of submitting offers.
