Execution of services in the field of solid and liquid waste disposal inNADACH administrated by GDDKiA Branch in Warsaw divided into 30 tasks:

Task No. 1 Faculty of Technology Road Laboratory for the export of solid waste,

Contractors who meet the conditions of competence or entitlements to conduct a specific professional activity may apply for the award of the contract, provided that this from separate regulations:

a) In the case of submission ofert for tasks: 1, 2, 4, 6, 7, 8, 10, 12, 13, 14, 15, 16, 17, 18, 19, 20, 21, 23, 25, 26, 27, 29, 30 requires that the Contractor should have an entry in the register in the commune in which he will collect municipal waste from property owners - the basis of art. 9c para. 1 of the Act of 13.9.1996 on maintaining cleanliness and order in communes (unified text Journal of Laws of 2016, item 250, as amended),

b)In the case of tendering for the task: 3 it is required that the Contractor has a permit to conduct activities in the area of freezing of septic tanks and transport of liquid waste with codes 14 06 02, 16 07 08, issued by the war, the mayor or the president of the city competent for the place providing services - in accordance with art. 7 par. 1 point 2) of the Act of 13.9.1996 on the maintenance ofcleanliness and orderliness in communes (consolidated text, Journal of Laws of 2016, item 250, as amended);

c) In the case of submitting bids for tasks: 5, 9, 11, 22, 24, 28 requires the Contractor shall have a permit to conduct activities in the area of freezing of septic tanks and transport of liquid waste, issued by a war, mayor or president of the city proper due to placese service provision - in accordance with art. 7 par. 1 point 2) of the Act of 13.9.1996 on maintaining cleanliness and order in communes (unified text Journal of Laws of 2016, item 250, as amended), d) Entry in the BDO Register in the scope of waste transport, in accordance with art. 49 par. 1 of the Act of 14.12.2012 on waste (Journal of Laws of 2018 item 21 and 2017, item 2422) concerns tasks: 1, 2, 4, 6, 7, 8, 10, 12, 13, 14, 15, 16, 17, 18, 19, 20, 21, 23, 25, 26, 27, 29, 30. In the case of Contractors jointly applying for the order, fulfillment of the condition referred to in point III.1.1) of the notice, each of the Contractors.

2 The contractor is obliged to submit a statement valid as of the day of submitting bids constituting preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the proceedings.

3. In the scope of Part IVIn this document, the Contractor only fills in the section "α"

4. The Contractor who refers to the resources of other entities in order to demonstrate the absence of grounds for exclusion and compliance with them, to the extent that they refer to their resources, conditions for participation in the proceedings, submits uniform documents also regarding these entities In the scope of Part IV of the single document, it should be completedonly section "α".

5 In the event of a joint application by the Contractors, a single document shall be submitted by each of these Contractors. They must be sent in accordance with the rules specified in IDW.

6 If the Contractor is established or resides outside the territory of the Republic of Poland Polish, and also when the person to whom a given document relates resides outside the territory of the Republic ofof Poland, the principles set out in - 7 and 8 of the Regulation of the Minister of Development of 26.7.2016 on the types of documents that the contracting authority may request from the contractor in the proceedings for the award of the contract (Journal of Laws of 2016, item 1126, as amended).

7. Contractor, within 3 days from the date of posting on the website the information referred to in Article 86 para. 5 at PPL, translationand to the Ordering Party a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 1 item 23 of Public Procurement Law - 8. The contractor is excluded from the contract award procedure in respect of which any of the circumstances referred to in Article 24 sec. 1 point 12 of the Public Procurement Law and art. 24 sec. 5 points 1, 2 and 4Pp.

9. The ordering party, according to arbar 24 aa of the Public Procurement Law, will first evaluate the offers and then examine whether the contractor whose offer has been evaluated as the most advantageous is not subject to exclusion and meets the conditions for participation in the procedure. The period of being bound by the offer referred to in point IV.2.6) of the announcement is 60 days. The period of being bound by the offer starts as soon as the deadline for submission of tenders expires.

11. Awardingshall not provide for the possibility of ordering referred to in Article 67 sec.1 point 6.

12. The deadline for completing the order for each part (task) has been specified in SIWZ
