Framework Agreement for the Supply of Linear Accelerators with Surface Imaging Guidance System and Dosimetry Equipment

Purpose of Supplies: Framework Agreement for the supply of linear accelerators with positioning guidance systems (patient andccessories) and dosimetry equipment for the quality control of treatments.

The DUME, whereby the tenderer declares that he is not in one of the following situations: exclusion referred to in articles 67 to 69 of the law of 17.6.2016.
The DUME, by which the economic operator declares that it satisfies the following selection criteria:
1) a decthe total turnover of the undertaking and the turnover of the area of activity covered by the market, covering the last 3 years available according to the date of establishment of the undertaking or beginning of the activities of the economic operator.
Visit of the required site and not to participate in the visit of the site causes the nullity of the offer: on pain ofthe bidder is obliged to make a site visit (lot 1 and lot 2). The tenderer attaches to his tender the attestation in annex correctly completed. During this visit, he will be able to verify the accuracy of the plans submitted (see Appendix L1 (Saint-Luc University Clinic), Schedule L2 (Saint-Pierre Ottignies Clinic), Schedule L3 (Saint-Jean Clinic)). Il will also note the difficulties and constraints of the work to be carried out as well as the methodology to be used and the safety rules to be respected.
A certificate of the site visit (see Annex B) will be established by the institution and given to the tenderer. at the end of the same visit.
He shall in particular verify that:
a) the material can be routed without problem to the place of installation.b) the weight of the equipment (empty, loaded, during tests ...) is compatible with the overload allowed by the ground.
The tenderer gets in touch with Mrs Francoise Vanneste (francoise.vanneste@uclouvain.be ) receipt of the specifications to schedule an appointment for the site visit for the Saint-Pierre Clinic and Saint-Luc University Clinics.
The Biddercontact Mr Ronnie Van Aelst (rvanaelst@clstjean.be) for the specifications in order to set up an appointment for the visit of the premises for the Saint-Jean clinic.
All the related studies (room layout) a the subject of the contract, wished by the tenderer will be in charge of the latter.
