IROP - Perinatology - Mobile Vacuum Cleaner

• The subject matter of the public contract is the delivery of 2 pieces of mobile extractor for the Gynecological and Obstetric Clinic of the General Faculty Hospital in Prague, including accessories, customs, transport installation, commissioning, initial validation, instruction according to the relevant provision of Act No. 268/2014 Coll. o zdravotnicke resources (further only Act No. 268/2014 Coll.), or. training the buyer and handing over the documents that are related to the delivered goods, especially the declaration of conformity and operating instructions in the Czech language in printed and electronic form, including the guarantee service and further according to other terms of the documentation. The subject matter of the public contract is delivery 2ks mobile extractor for Gynecological and Obstetric Clinic General Faculty Hospital in Prague, including accessories, customs, transportation, installation, commissioning, initial validation, instruction according to the relevant provision of Act No. 268/2014 Coll. on medical devices (hereinafter only Act No. 268/2014 Coll.), or training buyers and handing over documents to deliver goodsI also apply, in particular, the declaration of conformity and instructions for use in the Czech language in printed and electronic form, including warranty service and according to other terms and conditions of the tender documentation.

a) Direct link to which the entry in the Commercial Register at https: / /www.justice.cz/, if it is written in it, or written or a link to another similar record in which the supplier is registered according to Volthe Law of Legislation (Section 77 (1) of the Law). The supplier encloses the affidavit that the registered information is up to date and truthful. The supplier may also demonstrate the eligibility of the document by submitting the document itself in electronic form;

b) Direct link to which the authorized business can be verified according to special legal regulations to the extent that the subject of the public contract is http: //www.rzp.cz/, especially the document proving the respective trade license or license (Section 77 (2) (a) of the Act). The supplier encloses the affidavit that the registered information is up to date and truthful. The supplier may also demonstrate the eligibility of the document by submitting the document itself in electronic form;for distribution and servicing of medical devices (if medical device is offered according to Act No. 268/2014 Coll.). This registration must be valid as of the date of submission of the offer and the selected supplier must keep it valid for the entire time of fulfillment of the subject of the public contract. The supplier encloses the affidavit that the registered information is up to date and truthful. The supplier can prove danalso the proof of the document itself in electronic form. If the supplier has filed an application for registration but has not yet registered in the public register of SUKL, he / she must submit the application for registration in electronic form.

• The supplier does not have to submit the documents under letter a) and b) if the legal regulations in his country are similar professional competence is not required.
