The object of the contract is to complete the taskpn. "Development of technical conditions for launching the Center for Communication with the Citizen (CKzM) in the City of Toruń under the project:" Infostrada Kujaw and Pomerania 2.0 "co-financed by the European Union under the European Regional Development Fund under the Regional Operational Program of the Kuyavian-Pomeranian Voivodeship for the years 2014-2020.

Main Site: Municipality of Toruń, P.OLAND.

1.1. The subject of the order is the performance of the task "Development of technical conditions for launching the Center for Communication with the Citizen (CKzM) in the City of Toruń under the project:" Infostrada Kujaw and Pomerania 2.0 "co-financed by the European Union under the European Regional Development Fund under the Regional Operational Program of the Kujawsko-Pomors Voivodshipwhich for 2014-2020.

1.2. The subject of the contract includes the Contractor`s documentation, which includes in particular:

a) as part of stage I:

- "Diagnosis of CKzM operational and organizational area" report,

- "Diagnosis of ICT architecture for CKzM" report,

b) in as part of stage II:

- report "Definition of goals and main business assumptions for CKzM",

- report "Analysis oflegal provisions that have or may have an impact on the architecture and functionality of CKzM and the implementation process ",

- report" Business model of CKzM functioning ",

- report" Assumptions for the new UMT operational model,

- report "Detailed concept and requirements for CKzM",

- report "Recommendation of the best technologies for use in the authorization system of residents and communication channels",

- report "Model architecture of the ICT structure of CKzM at UMT,

c) as part of stage III:

- report "Assumptions regarding the implementation and operation of CKzM",

d) as part of stage IV:

- a set of tender documentation necessary to carry out the contracting procedure public in the mode of the Public Procurement Law Act for the Supply and Implementation of CKzM in particular: projectToR, formal requirements, admission criteria, evaluation criteria, draft contract for the delivery and Implementation of CKzM.

1.3. A detailed description of the subject of the contract is included in Annex 7 to the contract template.

1.4. The Purchaser does not allow submission of partial offers and does not allow the submission of Variant offers.

1.5. The Awarding Entity does not provide for exclusions under Article 24 paragraph 5 of the Public Procurement Law

1.6. The Awarding Entity does not provide for the requirements referred to in art. 29 paragraph 4 of the Act and making reservations referred to in art. 36 a paragraph 2 of the Act.

1.7. The contracting authority will use the reverse procedure, pursuant to art. 24aa of the Public Procurement Law

1.8. The awarding entity does not envisage awarding the contracts referred to in art. 67 section 1 item 6 of the Public Procurement Law

1. Contractors who can apply for the contract:

1) are not subject to exclusion;

2) meet the following conditions for participation.

2. In order to confirm that the conditions for participation in the procedure have been met and to demonstrate that there are no grounds for exclusion, the contractors shall submit the next offerimportant documents:

1) original EAT in electronic version;

2) commitment of entities to provide the Contractor with the necessary resources at the disposal of the contract, if the Contractor relies on the technical or professional capacity or financial or economic situation of other entities, regardless of the legal nature of the connecting go with them for legal relations;

3) Declarationwcy on belonging or not belonging to the same capital group.

3. In order to assess whether the Contractor relying on the capabilities or situation of other entities on the principles set out in art. 22a of the Act, will have the necessary resources to the extent enabling proper performance of the public contract and an assessment of whether the relationship between the Contractor and these entities guarantees a realaccess to their resources, the Employer requests documents that specify in particular:

a) the scope of resources available to the Contractor of another entity;

b) the method of using the resources of another entity by the Contractor in the performance of the public contract;

c) the scope and duration of the participation of another entity when performing a public contract;

d) whether entities, on the capacities which the Contractor relies in relation toand to the conditions for participation in the proceedings regarding education, professional qualifications or experience, perform works or services to which the abilities relate.

4. Other documents:

- the Contractor`s statement, which of the information contained in the offer on pages from ... to ... constitute business secret within the meaning of the Act on combating unfair competition andsuch may not be disclosed to other participants in the proceedings. If the Contractor reserves the information, he is obliged to demonstrate that the proprietary information is a business secret,

- the Contractor`s statement which part of the contract he intends to entrust to subcontractors and the information about the subcontractors` companies,

- proof of payment of the bid security,

- offer form with completed attachmentAnnex 2 to the bid form

Employer, in accordance with art. 25 paragraph 1 of the Act, will call the Contractor whose tender has been rated the highest to submit, within a specified period of time, not shorter than 10 days, the current fillings of statements or documents:

- information from KRK in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the Act, issued not earlier than 6 m. Before the expiry of the deposit dategiving offers,

- the Contractor`s statement that he has not issued a final court judgment or final administrative decision regarding late payment of taxes, fees or social security or health insurance contributions, or - in the case of such a judgment or decision - documents confirming payment of these amounts together with with interest or fines, if any, or concludedbinding agreement on the repayment of these receivables,

- the Contractor`s statement that there was no decision against him as a precautionary measure, prohibition of public procurement,

- list of services,

- list of persons referred by the Contractor to implement the public contract.

5. List of documents and statements to be provided by the National Contractor or the Contractor having its registered office or placeliving outside the territory of the Republic of Poland is in items 4.3.7- 4.3.10 of the SETC.

6. If the Contractor relies on the abilities or situation of other entities on the principles set out in art. 22a of the Act presents to the entities the document mentioned in point 4.3.1 of the ToR and the statements referred to in point 4.3.2 and 4.3.3 of the ToR.
