1. The subject of the order is the delivery of Chlorquinaldol. Form: lozenges.
2. Dose andthe quantity determines the form of the assortment price, constituting Annex. No. 1 to SIWZ.
3. Delivery time according to current needs, each time within a maximum of 2 business days from the date of order.
4. Series and expiry date on the unit packaging and applied to the invoice.
5. The validity period of the offered pharmaceutical product can not be shorter than 12 months from the date of deliveryto the Employer.
6. The pharmaceutical product offered must have admission to marketing and use in the Republic of Poland - for each required form and dose.
In order to confirm the absence of grounds for exclusion of the Contractor from participation in the proceedings:
1. Information from the National Criminal Register in the scope specified in art. 24 sec. 1 points 13, 14 and 21 of the Act, issued not earlier than 6 months before the expiry of theserminum for submitting offers or requests to participate in the proceedings.
2. Certificate of the relevant local organizational unit of the Zakład UbezpieSocial Insurance Fund or Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with paying social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of bids, or other document confirming that the Contractor has entered into an agreement with the competent authority in regarding the repayment of these receivables along withwith possible interest or fines, in particular he obtained the legally required exemption, postponement or spreading of overdue payments into installments or suspension in full of the implementation of the decision of the competent authority. to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 8 of the Act
4. A copy from the relevant register or central records and business information, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 1 of the Act.
5. Photocopy of a business license for a pharmaceutical wholesaler or a manufacturing permit (producers), issued by the Main Pharmaceutical Inspector, in accordance with the Pharmaceutical Law Act of 6September 2001, and in the case of the Contractor's contractor holding a warehouse permit containing the rights granted by the Main Pharmaceutical Inspector in the scope of trade in medicinal products, in order to confirm the fulfillment of the condition of participation in proceedings regarding entitlement to conduct a specific professional activity. Contractor's statements:
a. for lack ofon issuing to him a final court judgment or final administrative decision on overdue taxes, fees or social security or health insurance premiums or - in the event of issuing such a verdict or decision documents confirming payment of these receivables with possible interest or fines or conclusion of a binding agreement in repayment of those dueose
b. on the lack of a decision against him as a preventive measure and a prohibition against applying for a public order; about not paying taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2016, item 716).
In order to confirm that the offered deliveries meet the requirements specified by the Employer:
1 ) photocopies of documentsthe marketing authorization of the offered medicinal product, for each form and dose separately, in accordance with the provisions of the Act of September 6, 2001. Pharmaceutical Law (TJ Journal of 2016, item 2142 as amended);
2) CHPL of the offered medicinal product at the request of the Employer.
