Achizitie de aparatura medicala: aparat facoemulsificarepsilon, medical gas console, vaginal sonicator and abdominal probe, electrocautery with vascular sealing, advanced monopolar / bipolar electrocautery for open surgery, endoscopic hand washing and disinfection table, automatic endoscopic washing and disinfection machine, PACS, platform eye ultrasonography, complete videoendoscopy system for gastroenterology, optic coherence tomography, inner segment microsurgery kit according to the specification and the technical data sheets attached to the 12 batches. A supply contract will be concluded with an economic operator for one or more lots.

1 piece, according to the technical specification of the Tender Specifications.conomic must not be found in the situations provided by art. 164, 165, 167 of Law 98/2016. The economic operator (leader, associate, third party, subcontractor) will fill in the appropriate requirement in the DUAE form, according to art. 193 par. (1) of Law 98/2016. The framing of the provisions of art. 164, 165, 167 of Public Procurement Law 98/2016 entails the exclusion of the bidder inthe procedure applied for the award of the public acquisition contract; 2) Declaration on own responsibility for non-compliance with the provisions of art. 58 of Law 98/2016 on Public Procurement. In case of association, the qualification criteria for the personal situation will be met by each associate. The economic operator (leader, associate, third party, subcontractor) will complete the requirementcorresponding to the DUAE form and submitted to the SEAP together with it.

The manner by which the above mentioned requirements can be demonstrated: it will be completed by the economic operators participating in the award procedure with information related to their situation. The supporting documents demonstrating the fulfillment of the undertaken by completing the DUAE are to be presented at the(3) Certificate of fiscal attestation (including payment of social contributions) for legal entities, issued by the Ministry of Public Finance or its subordinate units, which have the status of budgetary creditor. Documentary evidence (tax certificates)updated to demonstrate the fulfillment of the qualification criteria, according to the information contained in the DUAE, which must attest the lack of overdue debts at the time of their submission. "4) Certificate regarding the payment of local taxes and taxes issued by the City Hall (Local Council).tax evasion) updated to demonstrate the fulfillment of the qualification criteria, according to the information contained in the DUAE; must attest the absence of overdue debts at the time of their deposit. Foreign legal / natural persons: editorial documents issued by the competent authorities of the country in which they are residentto prove the fulfillment of the outstanding obligations to pay the taxes to the state budget and the local budget, according to the legislation of the country of residence, valid at the moment of their submission. The documents shall be accompanied by their authorized translation into Romanian; 5) The judicial record of the economic operator and of the members of the administration body, management or supervision of the respective economic operator or of those who have the power of representation, decision or control within it, as it results from the certifying certificate issued by the ONRC / constitutive document; 6) According to the case, the documents by which demonstrates that the economic operator may benefit from the derogations provided for in art. 166 par. (2), art. 171 of Law no. 98/2016 regarding public procurement, 7) Other editing documents, as the case may be.

1) The way of defining possible offers with equal prices ranked first: if two or more offers have equal prices and are ranked on the same winning spot, in order to divide them, they will be required to re-enter SEAP (two decimal places).
