The subject matter of the contract is delivery of medical equipment (operating lights and lamps, sterilizing, disinfecting and hygienicapparatus) for the University Hospital with Policlinic Skalica, a. s., including delivery services, complete installation, functionality, operator training, guaranteed, safe service, removal and disposal of the delivery material.

Brief description of the subject matter of the vratan orderthe nature and quantity of the required goods is specified in point II.1.4) of this notice of public procurement.

1.1 The tenderer must fulfill the conditions of participation specified in - 32 para. 1 zakona č. 343/2015 Coll. on public procurement and amending and supplementing certain acts as amended (hereinafter referred to as "the Act").that the fulfillment of the conditions relating to personal status pursuant to - 32 para. 1 zakona, documents according to - 32 par. 2, respectively. pursuant to - 32 para. 4 and 5 zakona. According to - 32 par. 1 pism. (e) the tenderer must prove to the applicant that it is entitled to supply the goods covered by this contract. The lawfulness of a legal entity is required at the same time, according to Act no. 91/2016 Coll. on criminal liabilityof persons and on the amendment of certain regulations

1.2 Document according to - 32 par. 2 pism. e) prohibit the use of data from public administration information systems pursuant to a special regulation (Act No. 177/2018 Coll. on some measures to reduce administrative burdens by using public administration information systems and change and supplement some laws (against bureaucracy) nenai predkladať. 1.3 The Applicant registered in the List of Economic Entities under the Act is not obliged to submit documents pursuant to Section 32 (1) in the public procurement process. 2 zakona.

1.4 A person under - 8 prohibits the recognition of an equivalent record, such as being entered in the list of economic operators under the Act or confirmednot by a registration issued by the competent authority of another Member State to which the applicant proves compliance with the conditions for participation in a public contract. The person pursuant to - 8 shall also accept other equivalent documents submitted by the applicant.

1.5 The applicant may provisionally replace the documents proving the fulfillment of the conditions of participation specified by a person pursuant to - 8 of the Act by a uniform European document pursuant to - 39 of the Act. attestationof the Conditions of Participation is subject to the Act pursuant to Section 152 Para.

1.6 The vendor group demonstrates the fulfillment of the conditions for participation in the public procurement of a personal position for each member of the group separately. 8. Explanation of information needed for expthe bidding and demonstration of the fulfillment of the conditions of participation, the person under - 8 shall promptly notify at the latest 6 days before the expiry of the bid submission period, provided that an explanation is requested by the person in advance. Conditions of participation as notified, item III.1.1) Personal status, III.1.2) Economic and financial standing and Point III.1.3) Technical and professional capacity and meanstheir proofs are given in annex no. 5 SP. The model of the Single European Document is attached in annex no. 3 SP.

10. Bid security is required at EUR 6, 000.00. The method and conditions for the composition of the security are set out in accordance with the law and detailed in point 11 of the SP. I will prove to the bidders the bidding in the offer. The security must be credited to the designated account of a person under Section 8 of the Law or Submissionin the form of a bank guarantee in the tenderer's bid no later than the deadline for submission of tenders. A person under Section 8 of the Law shall classify the non-fulfillment of this requirement as a failure to comply with the conditions laid down by a person pursuant to Section 8 of the Law and shall exclude such claimant. The number of the account and the bank account for the deposit of the security is stated in the relevant section of the SP (point 11.5. A)). 12. Unjustification of the non-division of this contract into a part: The contract is not divided into parts because the subject matter of the contract is homogeneous from a functional, temporal, local point of view, also in terms of technical specifications and contract type.
