The subject of the public procurement includesdelivery, installation, commissioning, service training and warranty service of dental and operating equipment and ECG, pre-ordered and specified by type (commercial nomenclature), technical data, equipment and quantities (hereinafter referred to as "Apparatus" Equipment "or" Goods "in general) The goods are described in detail in Appendix 1 The Technical Specification of Documentsthe attentions. The subject of the public procurement is divided into 3 lots:
Main Site: 84, Tsar Osvoboditel Blvd., Medical and Dental Center - Supply, installation, commissioning, training for work and guarantee service of dental equipment under Lot No 1, detailed in the Technical Specification Annex № 1 of the documentation Total quantity - 11 pcmath units and 2 pcs. surgical units.- For lot 1 and 2 the participants shall hold a valid permit for wholesale trade of medical devices under Art. 77 of the Medical Devices Act, except in the cases under Art. 77, para. 2 of the MIR in which the participant is a producer established on the territory of the Republic of Bulgaria and may carry out commercial transactions with the products produced by them andarticles. An equivalent document issued by a recognized authority in the country where the participant is established will be accepted for participation by foreign persons.
For declaring compliance with the requirement, the participant should indicate the required information in Part IV: "Selection Criteria" A "Efficiency" by EEDOP, in cases where it is a manufacturer of the offered medical devices within the meaning of77, para. 2 of the Medical Devices Act, the participant declares this circumstance in the same section of EEDOP.
The documents for proof of compliance with the selection criteria are presented in accordance with Art. 67, para. 5 and para. 6 of the Public Procurement Act - certified by the participant a copy of a valid permit for wholesale trade of medical devices within the meaning of Art. 77 of the Medical Devices Act (or equivalent);the students of Art. 77, para. 2 of the Medical Devices Act declares a free text that it is a manufacturer of the medical devices offered
The Contracting Authority has no requirements regarding the criterion. 47, para. 10 in connection with Art. 39, para. 2 of the PPPP, a participant who submits an offer for more than one lot, can submit an EEDOP. For each lot is presenteda set of documents under Art. 39, para. 3, item 1 of the RPSP. The price proposal for each separate lot is placed in a separate opaque envelope with the inscription "Suggested price parameters" and the number of the lot is indicated. removes participants who are connectedand persons. The assignor shall remove from participation any participant for whom the grounds under Art. 54, para. 1 and Art. 55, para. 1, item 1 and item 4 of the Public Procurement Act, the conditions under Art. 107 of the Public Procurement Act and under Art. 3, item 8 of the Economic and Financial Relations Act with companies registered in preferential tax jurisdictions, the persons controlled by them and their actual owners. The assignor removesfrom the participant procedure for which the grounds under Art. Article 69 of the Law on Protection of Competition - Participant for whom there are grounds under Art. 54, para. 1 and Art. 55, para. 1, p. 1 and 4 of the Public Procurement Act shall have the right to provide evidence that he has taken measures that guarantee his / her reliability, despite the existence of the relevant ground for removal. 54, para. 1 andare grounds under Art. 55, para. 1, item 1 and item 4 of the Public Procurement Act and before submitting the tender, he has taken measures to prove the reliability under Art. 56 of the PPA, these measures are listed in Part III, Section A: "Grounds relating to criminal convictions" of EEDOP The following documents shall be presented in order to prove the lack of grounds for removal: for the circumstances under Article 54, paragraph 1, item 1 of the Public Procurement Act - certificate forconviction; for the circumstance under Art. 54, para. 1, item 3 of the Public Procurement Act - a certificate from the revenue authorities and a certificate from the municipality at the seat of the contracting authority and the participant; for the circumstance under Art. 54, para. 1, item 6 of the Public Procurement Act - a certificate issued by the bodies of the Executive Agency "Labor Inspectorate" for the circumstances under Article 55, paragraph 1, item 1 of the Public Procurement Act - a certificate issued by the Entry Agencytions; for the circumstances under Art. 55, para. 1, item 4 of the PPL - declaration. Where a tenderer selected as a contractor is a non-resident, he shall provide the relevant document issued by a competent authority under the law of the country in which he is established. The assignor does not require the submission of the documents proving the absence of grounds for removal when the circumstances areaccessible through a public free registry or the information or access to it is provided by the competent authority of the contracting authority on its own initiative.
The Assignor will use budget funds and own revenues. The payment will be made by bank transfer within 30 (thirty) calendar days after submission of delivery, installation, commissioning and training protocolssubscription and invoice
