The subject of the order in this tender is: delivery, installation and commissioning of medical equipment and apparatus for SP ZOZ Multospecialist Hospital in Jaworzno. The subject of the order was divided into 13 packages.

1) as appropriatetowards which the liquidation was opened, the arrangement approved by the court in the restructuring proceedings provides for satisfaction of creditors by the liquidation of its assets or the court ordered the liquidation of its assets pursuant to art. 332 paragraph 1 of the Act of 15.5.2015 Restructuring Law (Journal of Laws of 2015, item 978, 1259, 1513, 1830 and 1844 and of 2016, item 615) or whose bankruptcy was declaredzono, with the exception of the contractor who, after the declaration of bankruptcy, concluded the arrangement approved by a final court decision, if the arrangement does not provide for satisfaction of creditors by the liquidation of the assets of the bankrupt, unless the court ordered the liquidation of his assets pursuant to art. 366 section 1 of the Act of 28.2.2003 Bankruptcy Law (Journal of Laws of 2015, item 233, 978, 1166, 1259 and 1844 and of 2016, item 615).
Documents to be submitted with the offer.
1. price assortment lists (respectively on Annex 3.1 to 3.13, forms containing technical parameters offered by the Contractor and the Offer form prepared according to the model constituting Annex No. 1 to the ToR;
2. proof of payment of the bid security.
2. Together with the offer, the Contractor is obliged to send : current statement on the day of submitting bidsnolithic document (single European order document in abbreviation EAT), to the extent indicated by the contracting authority in the contract notice or in the Specification of Essential Terms of Order. The information contained in the statement constitutes preliminary confirmation that the contractor is not subject to exclusion and meets the conditions for participation in the procedure. The Single Document (EAT) is attached as Annex 2 to the ToR.
The Employer before awarding the order, will call the Contractor whose offer has been rated the highest to be submitted within a designated period, not shorter than 10 days, valid on the day of submitting the statements or documents confirming the circumstances referred to in art. 25 paragraph 1 of the Public Procurement Law (described in detail in Chapter V, section 8 of the ToR).
