1. The contractors who are allowed to order can apply for:
1.1. meet the conditions of participationin proceedings regarding competences or entitlements to conduct a specific professional activity, if it results from separate regulations. The ordering party does not specify any special requirements in this respect.
1.2. they are not subject to exclusion from the proceedings based on art. 24 sec. 1 and par. 5 points 1 and 8 of the Public Procurement Law, hereinafter referred to as Pzp
1.3. they meet the requirements specified in SIWZ. - 3.Each Contractor within 3 days of posting on the website (www.szpital-makow.pl) information referred to in art. 86 par. 5, provides the Employer with a declaration of belonging or non-affiliation to the same capital group, within the meaning of the Act of 16.2.2007 on consumer competition (OJ of 2015, item 1884, 1618 and 1634), with other Contractors whothey submitted offers. In the case of belonging to the same capital group, along with the submission of a statement, the Contractor may provide evidence (documents or information) confirming that connections with another Contractor do not lead to distortion of competition in the procurement procedure. - The Employer does not establish a specific condition for participation in this area .
