Periodic delivery of specific urology and endurology supplies for MMA needs

Periodic delivery of specific urology and endurology supplies for MMA needs, identified in 11 (eleven) separate positions.

Delivery of set stents for retrograde and antegrade ureteral splinting, suprapubic drainage and pecutaneous nephrostomy, separated into 40 nomenclature units.

In the column "Estimated total for all nomenclature units "- 350, detailed in technical specification -Annex No. 1, and technical proposal - Model No. 3 of the documentation.

The participant has received a wholesale marketing authorization for medical devices, issued by the BDA, according to the procedure of mass media or equivalent document.

Notes:

* Wholesale trade activities medical devices should be carried out by persons holding a valid wholesale marketing authorization for medical devices. These activities canalso be carried out by a person who has submitted a copy of a valid equivalent document proving his registration in one of the professional or commercial registers of the country in which he is established, or by submitting a declaration or certificate of the existence of such registration by the competent authorities under his national law .

The participant should provide permission information at haest IV, section A, item 1) of EEDEP, specifying number, date of issue and period of validity, this information is to be filled in item "Is this information available free of charge to database authorities in an EU Member State - there is pressed "yes" to open the field and to be able to enter the information necessary for the contracting authority.a participant to submit a copy of the authorization or equivalent document proving the information specified in the EEOP. A tenderer who offers a product (s) which does not meet the specific requirements of the contracting authority, detailed in the procurement documents, will be removed from the procedure for the specific lot.
