1. The subject of the order is the serviceie service for photocopiers located in all locations of the National Center for Agricultural Support, Regional Branch in Warsaw: 1) 00-095 Warsaw, Pl. Bankowy 2, 2) 01-424 Warsaw, Al. Prymasa Tysiąclecia 60/62, 3) 26-900 Kozienice, ul. Sportowa 2, 4) 06-407 Krasne, ul. Mickiewicza 36, 5) 81-110 Siedlce, ul. Poniatowski 81; 2. The ordering party requires employment of przz Contractor or Subcontractor persons performing directly activities related to servicing photocopiers, under a contract of employment within the meaning of the provisions of the Act of 26.6.1974 - Labor Code (Journal of Laws of 2016, item 1666); 3. A detailed description of the subject of the order is specified in the Specification of Essential Terms of Orders.
The Ordering Party does not set a condition in the above-mentioned order. in scope.
The ordering party does not put a wamarket in the above extent.
1. The ordering party, in accordance with art. 24aa of the Public Procurement Law, it will first evaluate the offers and then examine whether the contractor whose tender has been rated as the most advantageous is not subject to exclusion and meets the conditions for participation in the procedure. Contractors, who are not subject to exclusion based on the premises specified in art. 24 sec. 1 pkt 12-23 act and art. 24 paragraph 5 points 1.
3. Each Contractor must enclose with the tender the statements valid in the form of a single document (EED) valid at the day of submitting the offers in accordance with the standard form set out in the European Commission's implementing regulation of 5.1.2016 establishing a standard form for a single European order document., issued on the basis of art. 59 par. 2 of Directive 2014/24 / EU and art. 80 ust. 3 of Directive 2014/25 / EU;
4. In order to confirm the absence of grounds for exclusion from the proceedings for the award of a contract in the circumstances referred to in - 5 para. 1 point 1.1 of the SIWZ, will call the contractor whose offer has been rated the highest, to submit in the designated, not shorter than 10 days, date, current deposit dayof the following declarations and documents:
1) An excerpt from the relevant register or central records and information on economic activity, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion on the basis of art. 24 sec. 5 points 1 of the Act;
2) Information from the National Criminal Record in the scope specified in art. 24 sec. 1 points 13, 14 and 21 ujoints, issued not earlier than 6 months before the deadline for submission of bids or requests to participate in the proceedings;
3) Contractor's statements about the lack of a final court judgment or final administrative decision on default of taxes, fees or insurance premiums social or health care or in the event of such a sentenceor the decision of documents confirming the payment of these receivables together with any interest or fines or conclusion of a binding agreement regarding the repayment of such receivables (Annex 5 to item 1);
4) Contractor's statements about the lack of a ruling against him as a preventive measure for public orders (constituting Annex 5 tosiwz point 2);
5. Contractor within 3 days from the date of posting on the website information referred to in art. 86 par. 5 of the Act, will provide the orderer with a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 1 point 23 of the Act, in accordance with the template, which constitutes Annex 3 to the ToR. With the submission of the statement, the contractor maye provide evidence that links with another contractor do not lead to distortion of competition in the procurement procedure.
