Dremax. 225 l in the amount of 10 pieces. - Declarations, certificates, documents and documents pursuant to - 32 par. 2 of the Public Procurement Act, through which the applicant demonstrates fulfillment of the conditions for participation in public procurement related to personal status pursuant to - 32 par. 1 of the Act on Public Procurement. The interviewer / observer can prove that he / she fulfills the conditions of his / her personal status written in the listits economic subjects in accordance with Section 152 of the Public Procurement Act. Entry in the list of entrepreneurs carried out according to regulations effective until April 17, 2016 is entered in the list of economic subjects within the scope of recorded facts. The observer is deemed to be a satisfactory condition for the occurrence of a personal status under Section 32 1 letter. (b) and (c) of the Public Procurement Act, if he has paidarrears or was allowed to pay arrears in installments. Under - 152 para. 5 of the Public Procurement Act, the contracting authority is entitled to require the applicant or an additional person to request a document pursuant to - 32 par. 2 sheets. b) and c). The Applicant must also comply with the provisions of Act No. 91/2016 Z. z. on the criminal liability of the legal persons and on the amendment of some laws which are in forcearrived at 1.7.2016. As a result of this legislative regulation, economic entities with the status of legal persons have been created for the purposes of proving their personal status according to - 32 par. 1 letter. a) Act on public procurement obligation to submit also extract from the criminal record for the person entitled. An extract from the criminal record of a legal person is issued by the General Prosecutor's Office of the Slovak Republicrequest. If an applicant, a businessman, or his usual place of residence outside the territory of the Slovak Republic and the status of his place of business or place of business or usually resides, does not issue or issue any equivalent document, it may be replaced by an affidavit in accordance with the regulations in force in his / business or usual residence. If right of candidate status / za place of business or habitual residence outside the territory of the Slovak Republic does not regulate an institute of honesty, it may be replaced by pronouncements made before a court, a body, a notary, another professional institution or a commercial institution according to the rules in force in the place of business, place of business or usual residence of the applicant / candidates. In the case of a tenderer who makes a group of contractors involved in a public procurement, proof of compliance with the conditions of the applicant's personal status as a candidate for each member of the group is required. In case ofthe proposed price in the submitted bids or offers after the realization of the electronic auction will be higher than the estimated value of the bids on the subject of the bids, the contracting authority shall be further only as a person according to - 8 par. 1 letter. a) the Public Procurement Act reserves the right to indicate the offer as unacceptable and in the event that none of the offers is acceptable, it reserves the right to cancel the awardin conjunction with - 57 par. 2 of the Act on Public Procurement. Person under Section 8, Par. a) the Public Procurement Act reserves the right not to implement the object of procurement in the event that it submits an application for the provision of an unrecoverable financial contribution within the scope of OPVaI-MH / DP / 2017 / 3.3.1-08 and will not be approved and / or the result of the control of the VO procedure will violate the principles according to Acton Public Procurement and / or Legislation of the Slovak Republic and the EU. The detailed definition of the contractual terms and conditions for the supply of the required object of the order is made by the appendix no. 4 Procurements (Draft Purchase Agreement)

The tender documents will be requested by a written request in the Slovak or Czech language delivered electronically to the email address of the contact person mentioned in point I.3) of this noticeia, within the period referred to in point IV. 2.2) of this notice. The tender documents for the submission of applications shall be provided by the contracting authority exclusively in electronic form to the email address of the tenderer, as stated in the request for the submission of the contract documents. Explanation of the information necessary for the preparation of the offer, the proposal and the proof of compliance with the conditions for all-to the interested persons pursuant to Article 48 of the Act. The complete wording of the conditions of parting according to point III.1.) As well as the proof of their proofing are given in points 16.1, 16.2 and 16.3 of the tender documents. Documents proving the fulfillment of the conditions of participation in accordance with points 16.1, 16.2 and 16.3 of the tender documents may be replaced by the tenderer provisionally by means of the Single European Procurement Document (hereinafter referred to as onethe European document) in accordance with - 39 of the Public Procurement Act and according to point 16.4. of the supporting documents. Applicant in the formulation of a single European document according to appendix no. 5 of the tender documents shall fill in the required parts. From the submitted JED, it must be clear that the applicant fulfills the conditions of part time set out in point III.1 of this notice. In the case of communication, the contracting authority proceeds pArticle 187, par. 8 of the Act on Public Procurement, the term electronically means communication effected by e-mail. A successful bidder, prior to signing the purchase contract for the subject of the procurement, resulting from this procurement, will be required to provide the documents and documents according to point 30.8 of the tender documents. On the basis of - 11 par. 1 of the public procurement law does notto conclude a contract with an applicant or applicants who are required to register with the Public Sector Partners Register (RPVS) and are not enrolled in the RSPC or who are subcontractors or subcontractors under a separate regulation who have the obligation to write to the FVS and are not enrolled in the RCDS.
