Extracorporeal Lithotripter

The subject matter of the contract is the supply of Extracorporeal Lithotripter according to the technical specification which forms Annex A of the substrate. The extracorporeal lithotripsy kit consists of the following parts:

Main Site: Areal of L. Pasteur University Hospital Košice with its registered office at Rastislavova 43, 041 90 Košice, SLOVAKIAIKA. The exact pavilion will be specified when the deadline for the delivery of the goods referred to in point 1 of Article I of the contract is notified. The extracorporeal lithotripsy assembly consists of the following parts:

- Smooth wave generator

- X-ray machine with C-arm

- Intermediate table

- Control panel

Publicthe contracting authority requests the delivery of a set of extracorporeal lithotripsy in the amount of 1 pc. 1 of the Public Procurement Act. The applicant proves the fulfillment of the conditions of participation according to -32 par. 1 documents and documents referred to in -32 par. 2 Procurement Act

Detailed list of conditionsThe applicant can prove the fulfillment of the conditions for participation in personal status according to - 32 para. 1 pism. (a) to (f) by a valid entry in the list of economic operators maintained by the Public Procurement Office within the meaning of - 152 of the Public Procurement Act.showing compliance with the conditions of participation in the single European document pursuant to - 39.
