The subject of the order is the supply of dyeing reagents, reagents for histological and cytological treatment together with accessories, reagents and immunoreactants for immunohistochemical determinations together with accessories for the Department of Pathology and - supply of reagents, control materials and calibrators for research in the field of clinical chemistryperformed on the ABX Pentra C 400 ISE apparatus for the Analytical Laboratory

Primary staining reagents used in automatic dyeing and in manual staining methods. A detailed description of the subject of the order can be found in the Price Form / Order subject constitutingm attachments No. 1-1 to the ToR.

1. The Contractors, who are:

1.1 can apply for the award of the order. They are not subject to exclusion from the proceedings under Art. 24 sec. 1 item 12 23 and par. 5 point 8 of the Act on Public Procurement Law.

1.2 The Employer does not specify the conditions for participation in the procedure.

2. Prior to the awarding of the contractor, whose offer was rated the highest, at the request of the Employer -within the prescribed period, not shorter than 10 days - submit the documents valid as at the date of submission confirming the Contractor's exclusion from participation in the proceedings:

a) information from the National Criminal Register in the scope specified in art. 24 sec. 1 points 13, 14 and 21 of the Act on Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids,

b) certificate inthe competent head of the tax office confirming that the Contractor is not in default of paying taxes, issued not earlier than 3 months before the deadline for submission of bids, or other document confirming that the contractor has concluded an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, in particular he obtained the predictione the right to release, postpone or spread installments of overdue payments or suspend in full the decision of the competent authority, - c) a certificate of the relevant local organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with paying contributions social security or health insuranceotne issued not earlier than 3 months before the deadline for submission of bids or other document confirming that the Contractor has entered into an agreement with the competent authority regarding the repayment of these receivables with possible interest or fines, in particular obtained the lawful exemption, deferral or payment in installments of overdue payments or suspension in its entiretydecision of the competent authority, d) a statement by the contractor about the lack of a final court judgment or final administrative decision on default of taxes, fees or social security or health insurance premiums, or - in the event of such a verdict or decision - documents confirming the payment of these receivables with possible interest or mushrooms(e) a statement by the contractor about the lack of a ruling against him as a preventive measure for the prohibition of applying for a public order,

f) a statement by the contractor on non-payment of taxes and local fees referred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2018, item 1445).

3.If the Contractor is established or resides outside the territory of the Republic of Poland, instead of the documents referred to in point 11.2 of the ToR, he shall submit the documents specified in clause 11.3 of the SIWZ (www.kpcp.pl)

4. Detailed information on statements and documents submitted by Contractors confirming the absence of grounds for exclusion is contained in SIWZ (www.kpcp.pl). 1. The offer binding period is 60 days, lifrom the date of expiry of the deadline for submission of tenders.

2. The amount of the bid bond for each packet is specified in point 16 SIWZ (www.kpcp.pl)

3. The Contracting Authority does not provide for the possibility of making orders referred to in Article 67 sec. 1 point 7 of the Public Procurement Law.

4. The bid must be submitted for preliminary demonstration of the absence of any grounds for exclusion: ▪ Current statements as of the day of submitting bidsNot in the form of a Single European Order Document (EAT), in accordance with the guidelines contained in point 15 of the ToR.

5. Declarations and documents confirming no grounds for exclusion:

5.1 Pursuant to art. 24 sec. 11 of the Public Procurement Law. Contractor within 3 days from the date of posting on the website information referred to in art. 86 par. 5 provides the Employer with a declaration of affiliation ornot belonging to the same capital group as referred to in paragraph 1 point 23. Together with the submission of the statement, the Contractor may provide evidence that links with another Contractor do not lead to distortion of competition in the procurement procedure. The pattern of the declaration is attached as Appendix 4 SIWZ.

5.2 Prior to awarding the contractor, whose offer was rated the highest, onCalling the Ordering Party - within the prescribed period, not shorter than 10 days - will submit the documents confirming: 5.2.1 no grounds for exclusion of the Contractor from participation in the proceedings, which are specified in clause 11.2 of the SIWZ (www.kpcp.pl). If the Contractor is established or resides outside the territory of the Republic of Poland, instead of the documents referred to in point 11.2 of the SIWZ consists of: a) in the case of point 11.2.a) SIWZ - information from the relevant register or, in the absence of such register, another equivalent document issued by the competent judicial or administrative body of the country in which the Contractor has its registered office or place of residence has a person who is concerned with information or a document, to the extent specified in art. 24 sec. 1 items 13, 14 and 21 of the Acty Public procurement law, b) in the case of clauses 11.2 b) and c) of the ToR - a document or documents issued in the country in which the Contractor has his registered office or place of residence, confirming that he is not in default of taxes, fees, social security contributions or health, or that it has entered into an agreement with the competent authority regarding the repayment of such claims, together with any interestor fines, in particular obtained the legally required exemption, postponement or installment of overdue payments or suspension in full of the implementation of the decision of the competent authority, in accordance with point. 11.3 SIWZ (www.kpcp.pl).

Detailed information on statements and documents submitted by Contractors confirming the absence of grounds for exclusion and a list of documents and statements that must beto fine-enforce the Contractor to confirm that the offered item meets the requirements specified by the Employer, SIWZ (www.kpcp.pl).

6. Other documents and statements that must be included in the Contractor's offer.

6.1 Completed and signed price form (Annex 1 ... SIWZ) with the list of immunoreagents (Annex No. 7a, 7b)

6.2. Form filled and signed (Annex No. 2 SIWZ).

6.3 Proof of payment of deposit.

6.4. Power of attorney: attach the original or a copy authenticated by a notary if the Contractor is represented by a representative. According to art. 24aa of the Public Procurement Law, the Employer will not apply the reverse procedure.
