Delivery and service support of complex hospital information system and selected technical means for support of all processes and activities of public health care facility. The comprehensive hospital information system needs to fully cover all the needs of the patient-clinic system and also enable executives of the public procurerto track, evaluate and manage the organization's organization in real time to provide a comprehensive overview of the organization's public organization.

Main Site: Žilina University Hospital.

Implementation of the subject matter will include:

- delivery and installation of support system and database software, if the delivery and installation of the application programming equipment is requiredinstallation of specified technical means

- migration of existing central patient registry to the new system and import of outpatient findings and releases, findings from SVAL, RDG, new system pathology,

- migration of other relevant data from existing Medea StaPro system (their scope will be determined by needs within the project). The basic condition will be that the data isnecessary for the full performance of workers. Data that I will not need to migrate will not be

- data backup (eg dump generation, which will allow reliable system recovery)

- training of all users and administrators

- user support to start up

- project management

- system integration

- guaranteed service and guaranteed service

1.1. 1 prohibit the following documents:

a) pism. (a) evidenced by a statement of criminal records not older than 3 months on the date of expiry of the time limit for the submission of tenders. In connection with the acquisition of the Act no. 91/2016 Coll. on criminal liability of legal persons and on amendmentfrom 1 July 2016, the applicant shall also submit a copy of the list of criminals of non-legal persons not older than 3 months,

b) pism. (b) substantiated by an attestation by the health insurance company and the Social Insurance Agency of not more than 3 months at the date of expiry of the time limit for the submission of tenders; (c) evidenced by a local tax return not older than 3 months on the date of expiry of theie offers,

d) pism. (d) substantiated confirmation of the relevant keg not older than 3 months on the date of expiry of the time limit for the submission of tenders; (e) documented proof of entitlement to deliver the goods, carry out construction work or provide a service which corresponds to the subject of the contract; (f) by an affidavit,

The applicant or interested party shall not be obliged to submit documents under paragraph 1.2 if heThe contracting authority is entitled to use the information from the information systems of the trusted administration pursuant to a special regulation. If the applicant or applicant fails to submit a document under paragraph 1.2. font. a) is obliged to prove the condition under paragraph 1 p. (a) provide the PO with the information necessary to request the extract from the criminal record. The VO shall state in the notice of public procurement which the documents referred to in paragraph1.2 from the use of data from the public administration information systems do not submit.

1.6. The submission of originals or a certified copy of all documents is required.

On the grounds that the VO does not have access to the oversi.gov.sk portal the bidder submits all documents requested by VO in these tender documents. .The contracting authority requires a security of EUR 20 000.00.

2. The supporting documents, the communication, the provision of the explanation, the exchange of information are done electronically via the communication interface of the EVO system. All necessary information is provided by the contracting authority in the supporting documents.
