1. Przedmiotem zamowienia jest dostawand scientific equipment to the headquarters of the Institute of Electronic Materials in Warsaw, ul. Wolczyńskiej 133.

1. Contractors who are not subject to exclusion under Article 24 paragraph 1 may apply for the contract. 1 point 12) 23) of the Public Procurement Law and art. 24 sec. 5 point 1) and point 8) of the Public Procurement Law - 2. For the purpose of preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the proceedings, he must include akas at the day of submitting bids, a statement in the scope indicated in the SIWZ.

The declaration must be submitted on the form of the Single European Order Document attachment No. 5 to the ToR. The statement must be submitted in electronic form bearing the 'qualified electronic signature'. Specific requirements regarding the electronic form of EED are contained in Chapter X of the SIWZ.

3. In case ofjointly applying for a contract by the Contractors in the form of EAT, submit each of the Contractors jointly applying for the order.

4. The contracting party, prior to the order being awarded, will call the Contractor whose offer has been evaluated at the most, to submit within the prescribed (not shorter than 10 days) current date as of the day of submitting statements or documents confirming the circumstances, -referred to in art. 25 ust. 1 of the Public Procurement Law and Chapter VIII, para. 3 SIWZ

5. The Contracting Authority requires a deposit, the value of which has been specified in Chapter XI, para. 1 SIWZ.

6. The Contracting Authority does not provide for the conclusion of a framework agreement.

7. The ordering party does not intend to establish a dynamic purchasing system.

8. The Awarding Entity does not provide for the ordering referred to in art. 67 sec. 1 point 7 of the Pz actp.

9. The Contracting Party does not plan to conduct an electronic auction.

10. The ordering party does not provide for advance payments towards the order.

11. The Employer does not provide for the reimbursement of costs of contractors' participation in the procurement procedure, subject to the provisions of Art. 93 para. 4 of the Act

12. The orderer informs that before the commencement of the procurement procedure, not pa technical dialogue was carried out.

13. The Contracting Party declares that it does not intend to convene the meetings of the Contractors and does not plan to carry out a local vision.

14. The contracting authority does not limit the possibility of applying for a public contract only for contractors with more than 50% of employees being people with disabilities. The orderer does not reserve any part of the order, which can not be pcovered with subcontractors. Based on Article. 36b of the Act, the Orderer requires the contractor to indicate a part of the contract, the implementation of which is intended to entrust subcontractors and the contractor to give the names of companies (if known) - subcontractors.

16. The ordering party acting pursuant to art. 93 para. 1a may annul the procedure for awarding the order, if the means that the ordering party intended to allocateto finance all or part of the contract, they were not granted.

17. The offer binding period is 60 days. The period of being bound by the offer starts as soon as the deadline for submission of tenders expires
