The subject of the order is the provision of health services for the National Bank of Poland for the benefit of authorized persons.

1. The subject of the order is the provision of health services for the National Bank of Poland for the benefit of entitled persons.
2. As part of the services referred to in point 1, the Contractor undertakes to perform: - 1) health services for NBP Employees (Employee Package), including
The following benefits:
a) occupational medicine,
b) basic medical consultations,
c ) consultations with specialist opimedical staff,
d) outpatient procedures,
e) laboratory tests,
f) diagnostic tests,
g) home visits,
h) therapeutic and rehabilitation procedures,
i) pregnancy,
j) prophylactic dental care,
k ) one day surgery,
l) flu vaccination,
m) prophylaxis;
2) health services for people recruited to work at the NBP, including occupational medicine services in the field of- 3) health services for the NBP Employees' family member (Family Package and Affiliate Package), including the following benefits:
a) basic medical consultations,
b) consultations with specialist medical care,
c) outpatient procedures,
d) laboratory tests,
e) diagnostic tests,
f) home visits,
g) medical and rehabilitation treatments,
h) pregnancy care,
i) profiledental care, - j) surgery on one day, - k) vaccination against influenza, - l) prophylaxis, - 4) health services for pensioners / parents of the NBP, spouse
Emeryta / Rencisty NBP, parent of the NBP Employee / Life Partner of the NBP Employee ( Senior Package), including the following benefits:
a) basic variant:
i. basic medical consultations,
ii. specialist consultationsmedical care,
iii. outpatient procedures,
iv. laboratory tests,
v. diagnostic tests,
vi. home visits,
b) extended option:
i. basic medical consultations,
ii. consultations of specialist medical care,
iii. outpatient procedures,
iv. laboratory tests,
v. diagnostic tests,
vi. flu vaccination,
vii. home visits.
3. Information affecting the rea1) the National Bank currently employs 3 319 employees, including: a) 1 738 women in the age bracket:
- up to 30 67 people, from 31 to 40 years 287 people, from 41 to 50 586 people, from 51 to 60 years 686 people, over 60 years 112 people
b) 1, 581 men in the age range:
- up to 30 102 people, 31 to 40 years old 311 people, 41 to 50 years 494 people from51 to 60 years 501 people, over 60 years 173 people. C) by job because of the tasks:
- drivers - 39 people,
- bank guard employees (protection) 340 people,
- employees of the departments in cash -binary 322 people,
- employees of holiday and training centers - 56 people,
- Monetary Policy Council - 9 people,
- others - 2 553 people
2) now the number of emeraNBP employees and pensioners amounts to - 3 845 people. 888 people have a Senior Package, including: a) 533 pensioners and 355 parents of parents and parents of spouses, b) 619 women and 269 men, c) 13 people up to 60 and 875 people over 60.
1.The contractor is bound by the offer for a period of 60 days.
2.The period of binding the offer begins with the expiration of the deadline for submission of bids.
3. The bidder sets up a depositfor the entire subject of the order in the amount of PLN 200, 000.00. Specific requirements are contained in the Regulations.
4. Opening of bids: will take place at https://oneplace.marketplanet.pl at the NBP in Warsaw, ul. Jasna 8, on 24/05/2019 at 14: 00.
5.O contract may be awarded to Contractors who are not subject to exclusion from the proceedings under Art. 24 sec. 1 and par. 5 points 1, 2, 4 and 8 acts.
6. The contractor is obliged to show that he is not subject to exclusion from the proceedings.
7. In the event that the Contractor relies on technical or professional abilities of other entities, the Ordering Party will examine whether there are grounds for exclusion from other entities, referred to in art. 24 sec. 1 items 13-22 and para. 5 points 1, 2, 4
I 8 acts.
8. In the scope of confirmation of fulfillment by Wthe seller of the conditions of participation in the proceedings, the Ordering Party will demand the list referred to in par. 2 para. 4 points 2 and 3 of the Regulation of the Minister of Development of 26.7.2016 on the types of documents that the contracting authority may request from the contractor in the procedure for awarding the contract, hereinafter referred to as the Regulation on the types of documents - 9. In the absence of grounds for exclusion, the Ordering Party shall request, according tofrom art. 24 sec. 11 of the Act, the following documents:
1) declarations and documents referred to in par. 5 points 1, 4, 5, 9 of the Regulation on the types of documents, - 2) statements referred to in par. 5 point 10 of the Regulation on types of documents, - 3) documents and statements referred to in par. 7 and 8 of the Regulation on document types - 10. The ordering party in Annex No. 1 to RegThe application specified technical and organizational requirements for sending and receiving electronic documents and information transmitted using means of electronic communication.
11. Criteria for offer evaluation:
1) Offer price -73%,
2) Discounts -20%,
3) Number of own placements medicinal products available for NBP -7%.
12. The ordering party does not allow variant offers and does not provide for the orderreferred to in art. 67 sec. 1 point 6.
