• The subject of public procurement is the supply of EEG instruments for the JIP Neurological Clinic and Outpatient Department of the Children's and Adolescent Medical Clinic of the VFN in Prague, including accessories, duties, transport, installation, putting into operation, execution of instructions according to the relevant the provision of Act No. 268/2014 Coll. on Medical Devices (further only Act No. 268/2014 Coll.), -or training of the purchaser and delivery of the documents relating to the goods to be supplied, especially the declaration of conformity and instructions for use in the Czech language in printed and electronic form, including warranty service and further according to the other conditions of the accompanying documentation
The subject of the public procurement is the supply of the EEG device for JIP Neurological Clinics, including accessories, customs, transport, installation, uvcommissioned, execution of instructions according to the relevant provision of Act No. 268/2014 Coll. on Medical Devices (Act No. 268/2014 Coll. training of the buyer operator and handing over of the documents relating to the supplied goods, especially the declaration of conformity and instructions for use in the Czech language in printed and electronic form, including the guarantee service and further according to the other conditions(b) a document proving the right to business under special circumstances (Section 77 (1) of the Act); to the extent of the subject matter of the public offer, in particular a document proving the relevant trade license or license (- 77 par.ism. (c) a document attesting to its fitness for distribution and servicing of medical devices, ie one of the following options: - The confirmation of the registration of the registration shall be issued by the SLAK or - The extract from the SLAKL register pursuant to No. 268/2014 Sb. . on Medical Devices and on the amendment of the Act No. 634/2004 Coll., on Administrative Fees, as amended, or the URL address(d) certificate of authorization of the manufacturer for the distribution and servicing of the subject VZ
