In addition, Applicants must state the Applicant's insured amount in the applicant's valid professional liability insurance.

In the ESPD, the applicant must provide name, CVR no., mailing address, contact person, whether the applicant is a micro, small or medium-sized enterprise, and whether or not applicants participate in the tender procedure together with others (ESPD Part II.A). Furthermore, ans0gstate the name and address (s) of the person (s) who were authorized to represent applications in connection with this tendering procedure. Name, position, postal address, telephone, e-mail and necessary details of representation (ESPD part II.B) must be provided. The applicant must provide the requested information in Part III of the contract notice. The applicant must confirm, aApplicants are not covered by the mandatory grounds for exclusion in section 135 (3) of the Public Procurement Act. 1-3 and 136, see Implementation Order. - 10 (ESPD Part III, sections A, B and C). In addition, applicants must confirm that applicants are not covered by the voluntary grounds for exclusion in section 137 (1) of the Public Procurement Act. 1, No. 1, 2 and 5, see Implementation Order. Section 10 (ESPD Part III, Section C). The requested information formedis the basis for the Ordregiver's assessment of whether the applicant is fit to fulfill the task offered and which candidates are invited to submit tenders.
