1. Comprehensive construction project documentation.
Main Site: SP ZOZ MSWiA z W-MCO w Olsztyni
The subject of the orders in the present proceedings is:
1) execution preparation and handing over to the Ordering party a comprehensive multi-branch construction projecto, executive designs together with technical specifications for the execution and receipt of construction works, robot batch and investor cost estimates, guidelines for the safety and health plan, radiological cover design including the installation of the angiograph, obtaining opinions, approvals and permits, including building permits;
2 ) execution of a construction work involving a change in the wayboth use (reconstruction) of the G building premises for the needs of the Interventional Radiology Unit and Endoscopic Laboratory, in particular: general construction robot, sanitary robot in the field of plumbing, gravity ventilation, ventilation and air conditioning, central heating, medical gas installations, low voltage electrical installations and teletechnical;
3) delivery, installation, angiography, ultrasound, apparatus for anesthesia, anesthetic column and operating lamp, training of the Employer's personnel in the scope of servicing the above-mentioned devices, provision of maintenance services and apparatus maintenance during the warranty period.
Detailed description of the angiograph and other medical devices equipment contains attachment No. 2 toSpecification of essential order conditions Form summarizing the required technical and utility parameters. The functional and operational program referring to the execution of construction works is included in Annex 3.
Information on the processing of personal data
According to art. 13 para. 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27.4.2016 on the protection of individualsin relation to the processing of personal data and on the free movement of such data and the repeal of Directive 95/46 / EC (General Data Protection Regulation) (OJ L L 11.9, 4.00.2016, p. 1), hereinafter "RODO", I would like to inform you that: The personal data administrator is the Independent Public Health Care Center of the Ministry of Internal Affairs and Administration with the Warmian-Masurian Provincentrum Onkologii with headquarters in Olsztyn at al. Wojska Polskiego 37, POLAND; the inspector of personal data protection is Mr Jarosław Łęgowski, contact: e-mail address, [email protected]; personal data will be processed on the basis of art. 6 par. 1 lit. c ROPE for the purpose related to the public procurement procedure entitled: "Delivery of angiograph along with the change of the way of using parts of the roomfrom this building G through their reconstruction for the needs of the Department of Interventional Radiology and Endoscopic Laboratory "ZPZ-14/03/19, conducted under the open tender procedure, recipients of your personal data will be persons or entities, which will be available documentation based on the art 8 and 96 section 3 of the Act of 29.1.2004. Public Procurement Law (Dz.U. from 2017 item 1579 and 2018), hereinafter "PPL Act", after completing the order for your personal data will be stored for the period required by law. The obligation to provide your personal data directly to you is a statutory requirement set out in the provisions of the Act Pzp, related to participation in the public procurement procedurehim; the consequences of failure to provide specified data result from the PPL; in relation to your personal data, decisions will not be taken in an automated manner, application to art. 22 RODO; you have: pursuant to art. 15 RDO the right of access to your personal data relating to; based on Article. 16 RODO right to rectify your personal data **; based on Article. 18 RODO the right to request the administrator to restrict the processing of personal data, subject to the cases referred to in Article 18 para. 2 RODO ***; the right to complaint to the President of the Office for Personal Data Protection.
The Ordering Party does not place a specific condition for participation in the proceedings in this respect
This condition will be fulfilled if the Contractor will have a current, paid insurance policy fromcivil liability in the scope of business activity and liability for damage resulting from non-performance or improper performance of the contract, with a guarantee sum of at least PLN 2, 000, 000.00.
The ordering party requires a deposit of PLN 80, 000.
II. The contracting authority provides for the use of so-called the "reversed" procedure in accordance with Article 24aa of the Poviat Act - III. Zamaapplicant informs that pursuant to art. 93 par. 1a of the Public Procurement Law, the Orderer reserves the right to annul the proceedings for the award of the contract, if the funds that the Employer intended to allocate to finance all or part of the order will not be granted.
IV. The Contracting Authority requires a performance bond to be provided in the amount of 5% of the lump sum amountthe gross contractor.
V. Specific deadlines for order fulfillment -
1) deadline for completion of the subject of the Contract by 30.4.2020,
2) Submission by the Contractor of a material and financial schedule with a cost estimate used to control the process of contract performance and settlement of the robot submitted for the given moment to receive, within 5 business days from the conclusion of the contract,
3) thesermin completion of design works - construction project, 30 days from the date of contract conclusion,
4) deadline for project works - detailed designs, 20 days from the day of submitting the application for building permit,
5) handover of the construction site no later than 5 working days from the date of obtaining a building permit,
6) completion date of general construction works, sanitary installation works as well aselectrical and teletechnical installation works until 10/12/2019
VI. The Awarding Entity does not provide for supplementary orders referred to in art. 67 sec. 1 point 6 and 7 of the PPL and art. 134 sec. 6 points 3.
VII. The Contracting Authority informs that it does not intend to conclude a framework agreement, does not intend to establish a dynamic purchasing system, does not provide for the use of electronic auction. VII. Customer orderingz of the Contractors' proceedings based on the premises arising from art. 24 paragraph 1 point 12 -23 and art. 24 sec. 5 points 1 and 8 of the Public Procurement Law - VIII. In order to demonstrate the absence of grounds for exclusion, the Contracting Party requests from the Contractors to submit an EAT (initial declarations) and documents referred to in - 5 point. 1-6 and 9 and 10 of the Regulation of the Minister of Development of 26.7.2016 on the types of documents asthey can be requested by the contracting party from the contractor in the procurement procedure.
IX. In order to demonstrate fulfillment of the conditions for participation in the proceedings, the Contracting Party requests from the Contractors to submit the JEDZ and a document confirming that the Contractor is insured against civil liability in respect of the business related to the subject of the order for a guarantee sum of at least 2000, 000.00 PLN; list of deliveries made, and in the case of periodic or continuous services also performed, to the extent necessary to demonstrate compliance with the condition of technical and professional capacity during the last three years before the deadline for submission of bids, and if the period of activity is shorter - during this period, along with their values, subject, dates of completion and entities, onthe item of which the delivery was made together with the attached proofs confirming that the delivery was made or is carried out duly. This evidence, a list of persons referred by the Contractor to perform the orders responsible for managing construction works, along with information on their professional qualifications, qualifications, experience and education necessary to performthe scope of the public order, as well as the scope of activities performed by them and information about the basis to dispose of these persons. Detailed information on the above-mentioned documents and documents required in the proceedings are included in the Terms of Essential Terms of Purchase Orders. to SIWZ in accordance with the RODO Information opprocessing personal data.
The Purchaser informs that he has an account intended for electronic invoicing on the PEF expert Electronic Invoicing Platform, enabling the Contractor to submit an electronic invoice.
