1. The Contractor is obliged to submit a bid bond before the deadline for submission of tenders. The bid security: - for part 1 is: PLN 1, 000.00

- for lot no. 2 is: PLN 400.00 - for lot no. 3 is PLN 200.00 - for lot no. 4 is: PLN 20, 000, 00 PLN

- for lot no. 5: 800.00 PLN

- for lot no. 6 is: PLN 300.00

2. The offer must include the following education(in the form of an original or a copy certified by a notary public, provided that the document "EAT" must be submitted only using electronic means of communication (detailed in Chapter VI of the ToR)): a) Filled and signed by authorized representatives of the form the offer, prepared according to the template constituting Appendix No. 1 to the ToR.

b) EAT

- EAT- to enterconfirmation of the Contractor's compliance with the conditions of participation in the proceeding and the lack of grounds for exclusion, the Contractor shall submit the Single European Order Document (EIN), in accordance with EC (EU) 2016/7 executive management, dated 5.1 .2016 laying down the standard form of the Single European Order Document (EIN),

- In the case of joint application forfrom the contractors (consortium), a single document (EAT) is submitted by each of the contractors jointly applying for the order. These documents confirm the fulfillment of the conditions for participation in the proceedings, no grounds for exclusion, in which each of the contractors demonstrate compliance with the conditions for participation in the proceedings, no grounds for exclusion, - - if the Contractor, showing compliance with the conditions referred to in Chapter-V of the SIWZ refers to the resources of other entities in order to demonstrate the absence of grounds for exclusion against them and fulfillment of conditions, insofar as they relate to their resources, the conditions for participation in the proceedings also include EJ regarding these entities. C) Power of Attorney - to represent contractors in the proceedings or to represent the contractor in the procedure and conclusion of the contract, ifand the person representing the contractor in the procurement procedure is not indicated as authorized to represent it in the relevant register or business activity register. d) The power of attorney referred to in VIII. 2 and 3 SIWZ

e) Proof of payment / payment of a deposit / original guarantee.

3. List of statements and documents confirming compliance with the conditions of participation in the proceedings and bCanceling grounds for exclusion

3.1. Ordering prior to ordering, call the contractor whose offer has been rated the most, to submit in the prescribed, not shorter than 10 days, the current date as of the day of submitting statements or documents that confirm the fact of meeting the conditions for participation in the proceedings and lack of grounds for exclusion, ie: 3.1.1. To confirm independenceia excluding:

1) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submitting bids,

2) statements of the contractor about the lack of a final court judgment or final administrative decision on default of taxes, fees or social security contributionsor in the event of issuing such a verdict or decision documents confirming the payment of these claims together with any interest or fines or entering into a binding agreement on repayment of such debts; 3) declaring the contractor about the lack of a ruling against him as a preventive measure for the prohibition of applying for damages; public orders;

4) statements inthe executors about the affiliation or lack of belonging to the same capital group; in the case of being part of the same capital group, the contractor may submit together with the declaration documents or information confirming that connections with another contractor do not lead to distortion of competition in the proceedings (the Contractor submits this document within 3 days of posting on the websitej information referred to in art. 86 par. 5 p.z., - see Chapter V of the Terms of Reference, item 8).

3.2. If the contractor is established or resides outside the territory of the Republic of Poland, instead of the documents referred to in paragraph 3.1. point 3.1.1 (1)): - provides information from the relevant register or, in the absence of such a register, another equivalent document issued by the competent authority isor administrative department of the country in which the contractor has its registered office or place of residence, the person concerned by the information or document, to the extent specified in art. 24 sec. 1 items 13, 14 and 21 of the Public Procurement Law.

3.3 Documents referred to in paragraph 3.2, should be issued not earlier than 6 months before the deadline for submission of tenders or applications for admission to the companyin the proceedings.

3.4. If in the country in which the contractor has its registered office or place of residence or place of residence has the person concerned, the documents referred to in Chapter. paragraph. 3.2, it is replaced by a document containing the statement of the contractor, indicating the person or persons authorized to represent it, or a statement of the person whose document ist had to concern, filed before a notary or before a judicial, administrative or professional self-regulatory or economic authority competent for the place of residence or place of residence of the contractor or place of residence of that person. The provision of para. 3.3 shall apply.

3.5. The Contractor having its registered office in the territory of the Republic of Poland, in relation to the person residing inin the territory of the Republic of Poland, to which the document referred to in paragraph 3.1 point 3.1.1 item 1) of this chapter, submits the document referred to in para. 3.2 of this chapter, to the extent specified in art. 24 sec. 1 points 14 and 21 of the Act. If the country in which the person whose document is to be resident has his / her place of residence, no such documents are issued, it is replaced by a document fromnotifying a person's statement made to a notary or to a judicial, administrative or professional body competent for the place of residence of that person. The provision of para. 3.3 of this chapter shall apply accordingly.

3.6. Note:

The Contractor is not obliged to submit the documents referred to in paragraph 3.1 point 3.1.1 if:

- Contractorand will indicate the availability of declarations or documents confirming the circumstances referred to in art. 25 ust. 1 point 1 and 3 Pzp in electronic form at specific Internet addresses of free and public databases, in particular public registers as defined in the Act of 17.2.2005 on computerization of entities operating public tasks (Journal of Laws 2017, item 570). In this case, the Employer shall collect the statements or documents indicated by the Contractor individually from these databases. - The Contractor shall indicate declarations or documents confirming the circumstances referred to in Art. 25 ust. 1 point 1 and 3 PPL, which are in the possession of the Employer, in particular statements or documents kept by the Ordering Party in accordance with art. 97 ust. 1 uPzp. In this case, the Ordering Party, in order to confirm compliance with the conditions of participation in the proceedings and no grounds for exclusion, uses the statements or documents held, if they are up to date. Order mode: unlimited tender.

5. Foreseeable grounds for exclusion: art. 24 sec. 1 of the Act on Public Procurement Law. The Awarding Entity does not provide for exclusion under Article, 24 sec. 5th paragraph Pzp.

7. The Awarding Entity does not plan to conduct an electronic auction, establish a dynamic purchasing system or enter into a framework agreement.

8. Contractors may submit offers for any part of the order.

9. No technical dialogue was carried out.

10. Ordering in-proceedings will apply the procedure foreseen in art. 24aa of the Public Procurement Law. According to art. 24aa paragraph 1 Pzp, the Awarding Entity will first evaluate the offers and then examine whether the contractor whose tender has been rated as the most advantageous is not subject to exclusion and meets the conditions for participation in the procedure.

11. Documents and statements required by the Ordering Partysubmit the forms in the forms specified in the SIWZ and the announcement, with the reservation that the document JEDZ must be submitted only using means of electronic communication (more details are described in Chapter VI of the ToR).

12. Contract changes:

12.1. The Contracting Authority provides for the possibility of making changes to the provisions of the contract in relation to the content of the Contractor's offer, taking into account art. 144 of the Public Procurement Law, and in prin the event of:

1) changes in legal acts having a direct impact on the implementation of the subject of the contract,

2) changes in contractual remuneration in the event of a change in rates of tax on goods and services, in which case the contractual remuneration will be increased or reduced accordingly, - 3) subcontractor changes, if the Contractor in the offer has shown that he will rely on the knowledge and experience of other sub-brandsiotow, a change of such a subcontractor is only possible if the proposed subcontractor meets this condition; change of subcontractor must be justified by the Contractor in writing and requires written acceptance by the Employer, and the new subcontractor would not be subject to exclusion from the proceedings, as a result of which this contract is concluded,

4) change of address dataParties,

5) changes in terms and payment dates,

6) change in the manner of contract performance if the solutions proposed by the Employer or Contractor contribute to a higher quality of the final subject of the contract, and this change will not affect the Contractor's remuneration.

12.2. The party requesting to change the contract to the extent specified in paragraph 1 shall submit to the other Party an application for-change of the contract together with a written justification of the necessity to introduce such an amendment to the contract
