The subject of the orders is design and reconstruction of the technological part of the Hajdo sewage treatment plantin the biogas plant and other activities in the field of electric energy.
Main Site: Sewage Treatment Plant, Hajdow, Lublin, ul. Łagiewnicka 5, POLAND
The scope of works covered by the contract is:
I. Designing reconstruction of Sewage Treatment Plant Sewage Hajdow energy management (preparation and agreement with the Employer of complete design documentation for all necessary industries)with obtaining a final building permit).
II. Reconstruction of Sewage Treatment Plant Hajdow energy management, includes:
Expansion of the biogas desulfurization installation:
a) Delivery, assembly and commissioning of equipment together with the implementation of installations necessary for their proper functioning (biogas, drinking water, electric power supply, etc.): - second desulfurization plant biogas in the same technol• biogas dryer,
- installation for removal of siloxanes from biogas downstream of a dryer, made as a biogas filter with a coal bed
b) laying foundations for newly designed devices.
Other activities in the field of biogas electricity:
a) replacement of HAKnFtA 3 x 120 mm2 cable lines 6 kV supplying the pumps in the main pumping station from the R switchboard6 kV;
b) modernization of internal lighting of the following facilities: mechanical workshop, pumping station, main warehouse,
c) construction of a 40 kW solar installation on the roof of the blower building.
Note:
Gross price for designing the remodeling specified by the Contractor Sewage Treatment Plant "Hajdow" - energy management (preparation and reconciliation with the Employercomplete design documentation for all necessary industries together with obtaining the final building permit decision) can not exceed 6% of the gross price for the reconstruction of Sewage Treatment Plant "Hajdow energy management. In the situation when the gross price for designing the reconstruction of Sewage Treatment Plant "Hajdow" - energy management (preparation and reconciliation with the Ordering partyproject documentation for all necessary industries together with obtaining the final building permit) will be higher than 6% of the gross price for the reconstruction of Sewage Treatment Plant "Hajdow" - energy management, the Contractor's offer will be rejected.
The works included in the order have been divided into stages :
- Stage I - preparation and agreement with the Employer of the complete documentproject planning for all necessary industries together with obtaining the final building permit, including:
1) development and agreement with the Contracting Authority of the preliminary project.
2) development and agreement with the Contracting Authority of the construction project together with obtaining the final building permit decision.
3 ) elaboration and agreement with the Employer of the detailed design. - Stage II(construction works) Sewage Treatment Plant "Hajdow" - energy management, along with obtaining a permit for use, if it is required (in case when the use permit will not be required, the Contractor will notify the completion of construction and will receive confirmation of the notification of completion of construction and no objections to use).
Requirements of the Ordering Partyabout the subject of the order and the detailed scope of works are described in Volume III of the Terms of Reference: Description of the subject of the order - Functional and utility program (Descriptive part).
Requirements of the Ordering Party regarding Contract insurance are regulated in Volume II of this SIWZ model contract on public order

In order to confirm the fulfillment of the conditions by the contractorin the procedure or selection criteria regarding the economic or financial situation, the Employer requests the following documents: a) Profit and loss account, where the preparation of the report is required by the law of the country in which the contractor is established or residing, and if the financial statements are subject to audit by an auditor in accordance with the accounting regulationsalso with the opinion on the audited profit and loss account, and in the case of the Contractor not obliged to prepare the financial report - other documents defining the turnover (understood as net revenues from sales of products, goods and materials) for the last 3 financial years, and if the period of activity is shorter for the period from which you can getthe data referred to above, confirming the fulfillment of the condition referred to in point 9.1.2 a) IDW.
B) Information of the bank or cooperative savings and credit union, confirming the amount of funds held or the creditworthiness of the contractor, in a period not earlier than one month before the deadline for submission of bids, confirming the fulfillment of the condition referred to in pointkt. 9.1.2 b) IDW.
C) documents confirming that the Contractor is insured against civil liability in respect of conducted activities related to the subject of the order, confirming the fulfillment of the condition referred to in point. 9.1.2 c) IDW.
The full name of the order is: Design and execution of the construction works covered by Contract I / 3 "Reconstruction of the Hajdowo Wind Power Plant", falling within the scope of the Project entitled: "Expansion and modernization of the water supply and sewage disposal system in Lublin Stage III" Operational Program Infrastructure and Environment co-financed from the European Union funds 2014-2020
This procurement procedure is conducted under an open tender procedure in accordance with the Public Procurement Law in accordance withrt. 24aa of the Public Procurement Law, will first evaluate the offers and then examine whether the contractor whose tender has been evaluated as the most advantageous is not subject to exclusion and fulfills the conditions for participation in the procedure. The contractor may apply for the award of the contract, who will show no grounds for exclusion from the award procedure the orders referred to in art. 24 sec. 1 of the Public Procurement Law - for granting an orderpublic contractors may apply to contractors who demonstrate no grounds for exclusion from the procurement procedure referred to in Art. 24 sec. 5 Public Procurement Law in the scope of point 1) 2) and 4) 8). In order to demonstrate the absence of grounds for exclusion from the procedure for awarding the contract and confirmation of fulfillment of the conditions for participation in the proceedings, the Contractors shall submit the followingbut in the event of two or more Contractors jointly applying for the award of a contract, none of the Contractors may be subject to exclusion and this must be demonstrated for each of them. Single European Order Document (EAT) submitted in electronic form with a qualified electronic signature and then along with files constituting the offer ofresolved to a single archive file (ZIP) as a preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the proceedings, prepared and completed in accordance with the Regulation on the Single European Order Document in the scope compliant with Annex 2 to IDW

Contractor intendingtake part in the public procurement procedure, must have an ePUAP account. The maximum size of files sent via dedicated forms for: submission, modification, withdrawal of an offer or application and for communication is 150 MB. If the file with the offer is larger than 150 MB, divide the offer and send it in two or three separate files.the submission of offers, applications, notifications, electronic documents, statements or electronic copies of documents or declarations and other information is assumed the date of their transfer to ePUAP.
The Contractor submits the offer via the Form to submit, change, withdraw the offer or application available on ePUAP and made available also on the miniPortal. Identifier of proceedings and klpublic students for a given procurement procedure are available on the "List of all proceedings" on the miniPortal and on the Employer's website along with the ToR.
