Subject of contract no. 1 is a DX / CR Diagnostic Diagnostic Card composed of: CR / DX 3 Mpix Diagnostic Monitor, Diagnostic Monitor Graphics Card, and Workstation.delivery of 14 pieces of equipment and a guarantee period for equipment including accessories 5 years from the date of delivery. Installation and service are not part of the delivery

According to - 32 par. 1 Act no. 343/2015 Coll. on Public Procurement and on the Amendment of Some Conclusions as amended (hereinafter referred to as the Conclusion)or certified copies of all documents proving the fulfillment of the conditions for participation in personal status, which it proves pursuant to - 32 para. 2 law, respectively. ods. 4 a ods.
The economic operator may provisionally replace the documents proving the fulfillment of the conditions for participation by the Single European Document pursuant to - 39 of the Act.

1. When awarding the contract, the contracting authority determines the elelectronic communication, which is realized through the system of electronic public procurement (system EVO ver. 18) https://www.uvo.gov.sk/portal-systemu-evo-5f5.html. Applicant / candidate must be registered in the IS UVO for the economic entity, on behalf of which he submits the submission to the contract, which is published in the Profile of the University Hospital Bratislava on the portal UVO (https://uvo.govbr).
2nd The contracting authority carries out the award of the over-limit contract: Diagnostic stations pursuant to - 3 of Act no. 343/2015 Coll. on Public Procurement and on the Amendment and Supplementation of Certain Outcomes as amended (hereinafter referred to as the "Conclusion") in connection with - 66 para. 7 the second sentence of the law by the so-called. super reverse competition.
3. However, the explanation in this contract is done electronically, exclusivelythrough IS EVO. 6. The economic operator may provisionally replace the documents demonstrating compliance with the conditions for participationSingle European document pursuant to - 39 of the Act. Documents proving the fulfillment of the conditions of participation shall be submitted by the tenderer to the contracting authority according to - 55 par. Article 7 (1) of the Act shall, at the time and in the manner intended by the contracting authority. If the tenderer uses subcontractors in the performance of the contract (- 41), the winning tenderer will be obliged tobefore concluding the contract to prove that the subcontractor proposed by them fulfilled the conditions for participation in relation to personal status and that there were no grounds for exclusion under - 40 para. 6 pism. a) to h) and par. 7; the entitlement to supply the goods, to carry out the works or to provide the service shall be demonstrated in respect of that part of the object of the contract or the concession to be performed by the subcontractor.
