The subject of the contract is the provision of a health service in the conditions of the Nuclear and Disarmament Company, a.s. according to Act No. 355/2007 Coll. on the Protection, Promotion and Development of Public Health, as amended, the Decree No.208 / 2014 Coll. details of the scope and fulfillment of the performance of the work health service, -on the composition of the experts who carry it out and on the requirements for their professional competence in the wording of later regulations. Providing a work health service under the conditions of the Nuclear and Disarmament Company, a.s. according to Act No. 355/2007 Coll. about protection, podpore and the development of public health in the wording of the later regulations, Vyhl-ška č.208 / 2014 Z. z. details on the scope and fulfillment of the performance of the occupational health service, the composition of the experts to whom it is carried out, and the requirements for their professional competence in the wording of later regulations. The Applicant must comply with the conditions for the person's privacyreferred to in Section 32 1) zak. no. 343/2015 Coll. on Public Procurement and on the Amendment to Certain Laws, as amended (hereinafter referred to as the "Act"), they shall be required to prove them in accordance with Section 32 2) resp. (4) to (5) of the law by submitting the most advanced form of electronic form of documents and thereafter the applicant in the first place in the order shall be required to produce the original documents or their certified true copiesi, resp. within the meaning of - 152 of the Act, an applicant who is registered in the list of economic entities is not obliged to submit documents in order to prove the fulfillment of the conditions of his personal status according to - 32 par. 2) resp. 4) to 5) of the Act, in this case it is appropriate but not obligatory to submit only data on the valid entry in the list of economic entitiesthe Public Procurement Office, stating the registration number or identification data, on the basis of which it is possible to find it in this list. Entry in the list of entrepreneurs according to Act No. 25/2006 Coll. on public procurement and on the amendment of the laws in the wording of later regulations is entered in the list of economic subjects in the scope of recorded facts.
