Occupational Health Services for Employees in the Penitentiary System

The contracting authority will conclude a public procurement framework agreement for the provision of occupational health services for the employees of the penitentiary system for a period of 4 years, with only one economic operator, and the subsequent contracts will be concludedby the units of the penitentiary administration system.

1. Requirement: Offerors, third parties and subcontractors need notie to be found in the situations provided for in Articles 59-60, 164, 165, 167 of Law no.98 / 2016.

Requested Mandatory Requirement

Mode of accomplishment: the DUAE will be filled in by the economic operators participating in the award procedure with information about their situation. The supporting documents proving the fulfillment of the undertaken by completing the DUAE are to be presented atthe request of the contracting authority, only by the bidder ranked 1st.

2. Requirement: Fiscal attestation certificates regarding the absence of overdue debts in respect of the payment of taxes, duties or contributions to the general consolidated budget (local budget, state budget, etc.) at the time of the request

Requested Required requirement

-of performance: Will be presented to the DUAE, completing the sections related to the required requirements. For Romanian legal / physical persons will be presented supporting documents, respectively certificates confirming the payment of taxes and duties to the consolidated general budget and the local budget, and for foreign legal / physical persons will be presented editorial documents issued by the authoritiesin the country of residence, to prove the payment of outstanding debts to the state budget and the local budget in accordance with the legislation of the country of residence at the time of filing these documents. The aforementioned documents will only be presented by the tenderer ranked 1st at the request of the contracting authority upon completion of the assessment-of the bids.

NOTE:

The DUAE will be presented, completing the sections related to the requested requirements. For Romanian legal / physical persons will be presented supporting documents, namely certificates confirming the payment of taxes and duties to the consolidated general budget and the local budget, and for foreign legal / physical persons will be presented editorial documents issued by the authoritiescompetent in the country of residence to prove the payment of outstanding debts to the state budget and the local budget in accordance with the legislation of the country of residence at the time of filing these documents. The documents mentioned above will only be submitted by the tenderer ranked 1st at the request of the contracting authority upon completion of the evaluationand offers.

NOTE:

1. The failure of DUAE along with the offer (including the associate / third party supporter) entails rejecting it as unacceptable. The following documents will be filed with the DUAE: the firm commitment of the supportive third party to show the actual manner in which its support, subcontracting agreement and / or association agreement will materialize, as the case may be. Nedepuneretogether with the DUAE, is the basis for requesting clarification of possible inaccuracies in the form of the information contained in the DUAE sections of both the tenderer / candidate and the supporter / subcontractor, this being necessary to ensure the proper conduct of the Attribution
