Przethe subject of the order is a comprehensive cleaning service and the reception of the Ecological Education Center for a period of 24 months.

The subject of the order is the cleaning and cleaning of rooms inside the 3-storey office building at the headquarters of the Nadleśnictwa Janow Lubelski, ul. Bohaterow Porytowego Wzgorza 35, 23-300 Janow Lubelski and outside areas together with the supply of necessary materials, a detailed description of the subject of the order together with the list of activities and the frequency of performance is specified in Annex 1A to the ToR. The services will be carried out using your own equipment, media and materials to maintain cleanliness and hygiene measuresintended for daily restocking in toilets. Cleaning and disinfecting agents must have PZH (Polish National Institute of Hygiene) or safety data sheets and quality to ensure proper cleanliness.

Order size is as follows:

- office space cleaning - 1 800 m

- outdoor cleaning:

a) outdoor cleaning surfaces hardened ( pavements, squares, streets) including snow removalin winter - 11 700 m

b) outdoor cleaning surfaces green (flower beds, rebates and lawns) and mowing in the period from month IV to X - 25, 000 m

d) window cleaning (new windows) 2x in 500 m

e) washing carpets 1x in the year 100 m

f) mowing green areas 2 x in the year 17 900 m

g) nurturing (forming) hedge 599 mb

h) other works (moving furniture, conservation workk) - 100 hours during the year

i) wood preparation including fire and fireplace transport - 150 hours within one year

The value of the basic order without consideration of similar services and the option right was estimated at PLN 307, 616, 40 excluding VAT. The estimated maximum value from point II.2.6 includes the ordering of similar services as well as the possibility to complete the order in 120% (option right).

The ordering party does not make any particular requirements regarding the description of compliance with this condition. In order to confirm the lack of grounds for exclusion from the proceedings and confirm the conditions for participation in the proceedings. The lecturer will be obliged to submit a statement on the form of the Single European Order Document (EAT) which specimen specifies Reg. Implementing Commission (EU)

2. Evidence that servicesdemonstrated in the EED to confirm that the conditions for participation in the technical or professional capacity regarding the experience have been fulfilled or are carried out properly.

3. an excerpt from the relevant register or from central records and business information, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusionbased on Article. 24 sec. 5 point 1 of the PPL;

4. a certificate of the competent US governor confirming that the contractor is not in default of paying taxes, issued not earlier than 3 months before the deadline for submission of bids or other document confirming that the contractor has concluded an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, inpeculiarities obtained the legally required exemption, postponement or spreading into installments of overdue payments or suspension in full of the implementation of the decision of the competent authority;

5. certificate of the relevant ZUS or KRUS organizational unit or other document confirming that the contractor is not in arrears with paying social security or health insurance premiums, issued not earlier than 3 mprior to the deadline for submission of tenders or other document confirming that the contractor has entered into an agreement with the competent authority regarding the repayment of such receivables together with any interest or fines, in particular obtained legally permissible, postponement or payment of overdue installments or suspending in full the execution of the decision competent authority;

6.information from the CoRK within the scope defined in art. 24 sec. 1 point 13, 14 and 21 of the Act and, regarding a conviction for a prison sentence, to the extent specified by the ordering party under art. 24 sec. 5 points 5 and 6 of the Act, issued not earlier than 6 months before the deadline for submission of tenders;

7. a statement by the contractor about the failure to issue a final court judgment or a final administrative decision against himon the payment of taxes, social security or health insurance premiums or, in the event of such a judgment or decision, documents confirming the payment of such claims together with any interest or fines or a binding agreement on repayment of these debts (with respect to the exclusion described in Article 24 paragraph 1 point15 PZP)

8. attestation Lecture on the lack of a ruling against him as a preventive measure of a prohibition against applying for public orders (with reference to the exclusion condition described in Article 24 paragraph 1 item 22 PPL)

9. statement of the Lecture on the non-delivery of a final conviction of a conviction court for offense to a restriction of liberty or a fine to the extent specified by the awarding entity on the basis ofArt. 24 sec. 5 points 5 and 6 PZP.

10. statement of the Lecturer's failure to issue a final administrative decision against him in breach of his obligations under labor law, environmental law or social security regulations to the extent specified by the awarding entity pursuant to Art. 24 sec. 5 point 7 PZP

11. the contractor's statement about not paying taxes and fees loreferred to in the Act of January 12, 1991 on taxes and local fees (consolidated text Journal of Laws of 2018, item 1445, as amended). indicated in point 2-11 The Contractor will be required to submit within the deadline indicated by the Employer, not shorter than 10 days, specified in the invitation sent by the Contracting Authority to the Contractor after the opening of tenders.magan in the description of compliance with this condition of participation in the proceedings.
