Subject of the order is delivery of 4800 intelligent hybrid cards Gemalto Idprime MD 830 or equivalents with a transponder compliant with LEGIC CTC4096-MP or compatible communication with licenses for 70 positions of SafeNet Authentication Clients or equivalents adapted to offered cards.

1. Smart cards.

1) In part stycard:

Gemalto Idprime MD 830 card or equivalent that meets the following conditions:

- Native card support for Windows 7 and 10 systems in 32-bit and 64-bit, OSX, Linux and Android versions

- Standard CSP service and PKCS librariesa key)

- algorithms support at least: SHA-1, SHA-256, SHA-512

- Lifespan at least 450, 000 cycles of recording / cleaning

- Compliant with Base CSP standards Minidriver, IDGo 800 Minidriver, PKCS-with proximity readers working in the LEGIC Prime standard (Legic RF) and LEGIC Advant (ISO 1443A),

- 1 kB memory (Legic Prime MIM) and 3 kB (Legic Advant).

2. Licenses

The subject of the order is the SafeNet Authentication Client license, newer or equivalent, allowing full management of the delivered cards. Detailed information on licensing requirements can be found in the SIWZ.

8. If the Contractor is established or resides outside the territory of the Republic of Poland, instead of the document referred to in para. 3 point 3 lit. and the Contractor shall submit information from the relevant register or, in the absence of such a register, another equivalent document issued-by the competent judicial or administrative body of the country in which the contractor has its registered office or place of residence, the person concerned by the information or document, to the extent specified in art. 24 sec. 1 items 13, 14 and 21 of the Act

9. If the country in which the contractor is established or resides or the place of residence has a person who is a document, does not appeardocument referred to in paragraph 8 above, it is replaced by a document containing the Contractor's statement, indicating the person or persons authorized to represent it, or a statement of the person to whom the document was related, filed before a notary or before a judicial, administrative or professional self-regulatory or economic authority with regard to the seat or place of replacementthe execution of the person or place of residence of the person.

10. Contractor established in the territory of the Republic of Poland, in relation to a person residing outside the territory of the Republic of Poland, which is referred to in the document referred to in paragraph 3 point 3 lit. a, submit the document referred to in paragraph 8 within the scope specified in art. 24 sec. 1 points 14 and 21 of the Act. If in the country in which-the person to whom the document was to reside, no such documents are issued, it is replaced by a document containing a statement of that person made before a notary or before a judicial, administrative or professional self-governing or economic self-government authority. . Other requirements for required declarations and docsthe grounds are included in the SIWZ.

1. In confirmation of the fulfillment of the condition regarding participation in the economic or financial situation, the Contracting Authority requires that the Contractors have financial means or creditworthiness of at least PLN 200, 000.

2. The ordering party, before granting the order, will call the Contractor whose offer has been the highest rated, to submit in the designated, not shorter than 10 days, terpasses, valid as at the date of submitting the following documents:

- Information of the bank or cooperative savings and credit union confirming the amount of financial resources held or creditworthiness of the Contractor in the period not earlier than one month before the deadline for submission of tenders. The document mentioned in point 2 above is submitted in the original or a copy certified as compliantwith the original. The attestation of compliance with the original is made by the Contractor, the entity on whose capabilities or situation the Contractor or the Contractor jointly applying for the public order, in the scope of documents which concern each of them.

4. Detailed information is contained in the ToR.

1. The Contracting Party requires a security deposit in the amount of PLN 7, 000 (in words: seven-thousand zlotys). Specific requirements in this respect are specified in SIWZ.

2. Contractors who are not subject to exclusion from the proceedings under Art. 24 sec. 1 point 12-23 of the Act on Public Procurement Law (Journal of Laws of 2017, item 1579, as amended).

3. On confirming compliance with the conditions referred to in paragraph 2 Contractors are submitting:

1) Actual on the day of their submissionia offers a statement in the scope indicated by the Ordering Party in the SIWZ, in the form of a single document (EAT). The Contractor shall send to the ordering party electronically encrypted and signed by a qualified electronic signature of the EAT to the email address bzp@msz.gov.pl, in such a way that the document reaches the orderer before the deadline for submission of tenders. The Employer provides the form JEDZ on pyour website. Detailed information on EAT and how to fill it in and submit it are in the ToR.

2) Contractor within 3 days from the date of posting information on the website of the Employer, referred to in Article. 86 par. 5 of the Act (information from the opening of offers), is obliged to provide the Employer with a declaration of belonging or non-membership-to the same capital group as referred to in art. 24 sec. 1 point 23 of the Act, with Economic Operators who have submitted separate offers in the proceeding. Detailed information in this regard can be found in the ToR.

3) The contracting party, before awarding the contract, will call the contractor whose offer has been rated the most, to submit within the deadline, not shorter than 10 days, valid for the daysubmit the following declarations or documents:

a) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submitting offers;

b) a statement by the contractor about the lack of a final court judgment or final administrative decision on late payment of fees, fees or-social security or health insurance premiums, or - in the event of such a judgment or decision being issued - documents confirming the payment of such claims together with any interest or fines or the conclusion of a binding agreement regarding repayment of such claims. The model of the declaration is attached as Appendix 7 to the ToR,

c) the contractor's statement about the lack of a ruling against him with the title of Wednesdaypreventive prohibition of applying for public orders. The pattern of the declaration is attached as Appendix 7 to the ToR.

4. The statements referred to in para. 3 point 2 and point 3 lit. b and c are put in the original.

5. Documents mentioned in paragraph 3 point 3 lit. and they are submitted in the original or a copy certified for compliance with the original. Confirmation of compliance with the original is made by the Contractor, as appropriatethe situation in which the Contractor or the Contractor jointly applying for the award of a public order is involved, as regards the documents that each of them pertains to.

6. In the case of Contractors jointly applying for the order, statements and documents listed in paragraph 3 consists of each of these Contractors

7. In the case of reliance by the Contractor on resources or the situation of other entitieslitter, the Contractor submits the statements and documents listed in paragraph 3 point 1 and point 3 also submit each of these entities.
