Reference Number: SUSCCH-NZ-VS-2019-implantable devices The subject of the contract is the delivery of implantable devices (cardiac pacemakers and ICDs with a completeaccessories including electrodes) for electroimpulse therapy of heart rhythm disorders and heart failure for the contracting authority listed in Annex no. 4 competition documents. The subject of the contract is divided into 9 (in words: nine) sections. The breakdown of the subject of the contract into parts and a description of the required minimum characteristics of the individual items of the subject of the subject of the contract is given in Annex no. 4 competition documents.Box 1 of the part must be submitted for all the relevant items. The offered goods must be found in the database of medical aids maintained by the State Institute for Drug Control. The subject of the contract must include the following services: transport of the subject of the contract to the places of performance referred to in point A-6.1.

Subject of Part 1 is delivery of goods: Implantable devices (cardiac pacemakers, ICDs and CRTs with complete accessories including electrodes) for electro-pulsotherapy of heart rhythm disorders and heart failure with special regard to:

- Possibility of continual (beat-to-beat) verification of pacing capability and automatic optimizationstimulation expenditure

- automatic adaptive sensitivity level adjustment after stimulated and spontaneous events in ventricles and atria,

- support MRI mode for high resolution MRI (3T and 1.5T at SAR 2W / kg) for selected pacemakers and defibrilators

- Possibility of stimulation of several places in the left ventricle, including services listed in Part C-P and in point II.1.4.) Of this notice

A detailed specification of Part 1 of the subject-matter of the contract and a description of the required minimum characteristics of each item can be found in Annex no. 4 to SP. Box 1 of the part must be submitted for all the relevant items. The offered goods must be found in the database of medical aids maintained by the State Institute for Drug Control.fulfill the conditions of participation according to - 32 par. 1 of the Public Procurement Act. They shall prove their fulfillment according to - 32 par. 2 of the Public Procurement Act by submitting scanned original documents or certified copies thereof. With regard to the wording of the Methodological Guidelines of the Public Procurement Office no. 5151-5000 / 2019 of 5.1.2019, the contracting authority informs candidates / tenderers thatthe contracting authority is not entitled to obtain documents to prove the fulfillment of the conditions for participation in personal status from public administration information systems; 1 of the Act, the documents referred to in - 32 para. 2 zakona.The applicant can prove fulfillment of the conditions of participation by enrollment in the list of economic subjects according to - 152 of the Act on Public Procurement. posted on https: // www.uvo.gov.sk/zazcacachadzac/registers-of-the-health-subjects-guided-of-the-year/information-to-the-list of-the-health-subjects-2ff.html, by which the UVO gives the economic operators attention to adjusting the conditions for entry in the list of economic entities resulting from Act No. 343/2015 Coll. on public procurement, which came into effect on 18 April 2016.

The tenderer may, pursuant to - 39 of the Publicprovisionally replace the documents proving the fulfillment of the conditions of participation specified by the contracting authority by means of the Single European Document (hereinafter referred to as JED). JED for the purpose of the Public Procurement Act is a document by which the economic operator can pre-replace documents proving the fulfillment of the conditions of participation determined by the contracting authority.m, by JED, in order to prove the fulfillment of the conditions for participation specified by the contracting authority, to fill in the Globalny data for all conditions for participation.
