Contractors who can prove the lack of grounds for exclusion under art . 24 section 1 items 12-23 and art. 24 paragraph 5 points 1 PPL. 1) Documents afterclaiming no grounds for exclusion: a) declaration of no grounds for exclusion, prepared according to the template constituting Annexe 1a to the ToR, in accordance with the Instruction of completion (unified order document EAT www.uzp.gov.pl) to the extent specified in SIWZ - submit, under pain of nullity, in electronic form, and provide a qualified electronic signature; Attention!!! Rulesfile signing, EAT certified (signature), file security rules, with the password included in Annex 1b to the ToR,

b) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the PPL, issued not earlier than 6 months before the deadline for submitting bids;

c) excerpt from the relevant register, if separate regulations require an entry in the-register or records to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 1 PPL in the event of the inability to download a document from the website indicated by the Contractor;

d) statement of the Contractor about the lack of a final court decision or final administrative decision on default of fees, fees or insurance premiums-social or health, or in the event of issuing such a verdict or decision documents confirming the payment of these receivables with possible interest or fines or conclusion of a binding agreement on repayment of such receivables; e) declaration of the Contractor about the lack of a ruling against him as a preventive measure of the prohibition to apply for public orders;

f) declarethe contractor to belong to or not belonging to the same capital group; in the case of belonging to the same capital group, the contractor may submit together with the statement documents or information confirming that connections with another contractor do not lead to distortion of competition in the proceedings. The Contractor shall give the ordering party (without additional request) within 3 days ofd on the day of providing the information referred to in art. 51 par. 1a, art. 57 par. 1 or art. 60d para. 1 or from posting information on the website referred to in art. 86 PPL above statement (template Annex No. 4 to the Terms of Reference).

2) Documents confirming compliance with the conditions of participation in the proceedings: a) declaration on compliance by the Contractor with the conditions specified in the Terms of Reference, and required onArt. 22 ust. 1 point 2 of the PPL, prepared according to the template constituting Annexe 1a to the Terms of Reference in accordance with the Instruction of completion (unified order document EAT www.uzp.gov.pl) to the extent specified in ToR must be submitted, under pain of nullity, in electronic form, and have a qualified electronic signature ; Attention!!! The rules of signing the files EVER certify (signature) the qualificationThe list of services (template Annex No. 5 to the Terms of Reference) confirming the condition described in Chapter 7 is included in the SIWZ Annex No. 1b to the SIWZ. III pt. 2b, performed, and in the case of periodic or continuous benefits also performed, within the last 3 years before the deadline for submission of tenders, and if the period of operation-is shorter in this period together with their value, subject, date of performance and entities for which services have been performed, and attaching evidence determining whether the services have been performed or are performed properly, while the evidence referred to are references or other documents issued by the entity for which the services were performed, and in the case ofborderline or continuous are executed, and if for a justified reason of objective nature the contractor is not able to obtain these documents - a statement by the contractor;

The offer must be secured by a bid bond. The ordering party will retain the deposit, if the premises mentioned in art. 46 ust. 4a and 5 PPL.

The bid bond must cover the entire period of being bound by the offer.

The Contractor's offer shall not be securedwith this form of deposit, will be rejected by the Employer.

By entering the tender for the entirety of the order, the contractor is obliged to pay a deposit in the amount of PLN 14, 000.00 (say: fourteen thousand zlotys, 00/100). ie 23/11/2018 at 10: 30

2. Form of deposit payment - the bid bond can be paid in the following forms:

1) suretyiach bankowe or socially-credit savings bond sureties, except that the surety is always a cash guarantee,

2) bank guarantees,

3) insurance guarantees,

4) or guarantees specified in art. 45 par. 6 PPL,

5) by transfer to the account of the Employer - funds should affect the account of the Employer by November 23, 2018 to 10:30 under pain of rejectionoffer to be canceled. Bank Gospodarstwa Krajowego O / Wrocław No. 07 1130 1033 0018 7991 8520 0007 with the following information: 'Wadium in unlimited tender, case mark: 92 / WAO / 2018'.

Note: first accounting session at Bank Gospodarstwa Krajowego O / Wrocław The Contracting Party's account takes place after 10.00 am On the recognition by the Employer that the deposit in cash has been paid within the required time, decidesdate and time of the influence of the funds on the Employer's account.

3. If the Contractor submits a security deposit in the form of a guarantee, the guarantee must be irrevocable, unconditional and payable within 30 days at the first written request of the Ordering Party, must be enforceable on the territory of the Republic of Poland, prepared in accordance with applicable law and contain the following elements: ) The name dthe order (Contractor) of the beneficiary of the guarantee (the Employer), the guarantor (bank or insurance institution providing guarantees) and indication of their headquarters;

2) Determination of the claim to be secured by the guarantee;

3) Guarantee amount;

4) The validity period of the guarantee;

5) Obligation of the guarantor (bank or insurance company) to pay the entire amount of the bid security irrevocably or beconditionally, at the first request of the Employer (beneficiary of the guarantee),

4. The offer must be accompanied by the original proof of payment of the deposit (transfer) or generated electronic confirmation of the transfer (document prepared under Article 7 of the Act of 29 October 1997 on Banking Law (Journal of Laws of 2015, item 128) requiring a signature or a stamp).

5. In the case of a deposit by Wthe contractor in other forms, the original document confirming the payment of the bid bond should be submitted to the deposit with the Main Accountant Hospital (Hospital Fund, General Administration Building), and a photocopy confirmed for compliance with the original attach to the offer.

6. It is not allowed to deposit a bid bond in a currency other than PLN, this provision also applies to deposits deposited in a form other than cash.

Uwaga! Filing a wageable document in a different place and incorrect form may result in the application of the sanctions resulting from art. 89 paragraph 1 point 7b of the Public Procurement Law.

7. The return of the bid bond or its possible re-submission is governed by the provisions of art. 46 and art. 184 Pzp.
