The subject of the contract is the performance by the Contractor for the Employerfor comprehensive public relations services along with ecological education in the implementation of the project entitled Construction of the Municipal Thermal Waste Transformation Plant in Gdańsk, through the provision of permanent services and ad hoc services.

The subject of the contract is the provision by the Contractor of comprehensive public relations services with environmental education for the Employerduring the implementation of the project entitled Construction of the Municipal Waste Thermal Conversion Plant in Gdańsk by providing permanent services, as described in chapter 7.1 OPZ (Annex 9 to the ToR) and ad hoc services, the catalog of which has been described in chapter 7.2 OPZ (Annex 9 to the ToR), provided as required by the Employer. The amounts of individual ad hoc services indicated in the OPZ constitutemaximum amounts that the Purchaser may order during the contract period. Contractors will not be entitled to any claims against the Employer in the event of the commission of a smaller number of individual ad hoc services.ci is shorter in this period, the average net sales revenue of at least PLN 1, 000, 000 per year.

1. Contractors are excluded from the procurement procedure in the circumstances referred to in art. 24 paragraph 1 of the Act.

2. The Employer provides for the exclusion of the Contractor pursuant to art. 24 paragraph 5 points 1, 4 and 8 of the Act.

3. To confirm that there are no grounds for excluding the Contractor fromparticipation in the proceedings. The Awarding Entity requests the following documents:

1) information from the National Criminal Record in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids;

2) a certificate from the competent head of the tax office confirming that the Contractor is not in arrears with tax payment, issued not earlierless than 3 months before the deadline for submission of bids or other document confirming that the Contractor has entered into an agreement with the competent tax authority regarding the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or installment of overdue payments or suspension in full of the decisionego body;

3) a certificate from the competent organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submitting offers, or another document confirming thate The Contractor has entered into an agreement with the competent authority regarding the repayment of these receivables, together with any interest or fines, in particular, he obtained the exemption, deferment or installment of overdue payments as provided for by law or withholding in full the execution of the decision of the competent authority;

4) an excerpt from the relevant register or from central records and information on business activitiesj, if separate provisions require an entry in the register or a register in order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 item 1 of the Public Procurement Law;

5) the Contractor`s statement that he has not issued a final court judgment against him or a final administrative decision on late payment of taxes, fees or social or health insurance contributions, or in the case ofand such a judgment or decision of documents confirming the payment of these receivables together with any interest or fines or the conclusion of a binding agreement on the repayment of those receivables;

6) the Contractor`s statement that there was no decision against him as a preventive measure prohibiting him from applying for public contracts;

7) Contractor`s declaration of non-payment of supplylocal taxes and fees, referred to in the Act of 12 January 1991 on local taxes and fees;

8) the Contractor`s statement on belonging or not belonging to the same capital group; in the case of belonging to the same capital group, the Contractor may submit, together with a statement, documents or information confirming that links with another Contractor do not lead toabout distortion of competition in the proceedings.
